血清sLR11、ESM-1及AGEs在子痫前期患者中的表达及其预测不良妊娠结局的价值

Predictive value of serum sLR11,ESM-1 and AGE expression in preeclampsia patients and their ability to predict adverse pregnancy outcomes

摘要 目的探讨子痫前期(PE)患者血清可溶性低密度脂蛋白受体11(sLR11)、内皮细胞特异分子-1(ESM-1)及晚期糖基化终末产物(AGE)的表达及其预测不良妊娠结局的价值。方法选取2020年1月至2022年10月张家口市妇幼保健院收治的141例PE患者(PE组)和60例产检正常孕妇(对照组),检测各组血清sLR11、ESM-1及AGE水平。根据PE患者严重程度分为轻度子痫前期(MP)78例和重度子痫前期(SP)63例,根据PE患者出现不良妊娠结局情况分为不良妊娠结局组57例和妊娠结局良好组84例。绘制ROC曲线分析血清sLR11、ESM-1及AGE水平预测PE患者不良妊娠结局的价值。Pearson相关分析PE患者血清sLR11、ESM-1及AGE水平之间的相关性。结果PE组血清sLR11、ESM-1及AGE水平均明显高于对照组(均P<0.001)。SP组血清sLR11、ESM-1及AGE水平明显高于MP组(均P<0.001)。不良妊娠结局组血清sLR11、ESM-1及AGE水平均明显高于妊娠结局良好组(均P<0.001)。ROC曲线显示,sLR11≥11.65μg/L、ESM-1≥2.14μg/L及AGE≥57.38 ng/ml三项联合预测PE患者不良妊娠结局的曲线下面积(AUC)最大(0.947,95%CI:0.890~0.995),其灵敏度为97.3%,特异度为82.0%。相关分析显示,PE患者血清sLR11、ESM-1、AGE之间均呈正相关(均P<0.001)。结论PE患者血清sLR11、ESM-1及AGE水平明显升高,与疾病严重程度密切相关,三项联合预测PE患者不良妊娠结局具有较好的价值。 Objective To investigate the serum levels of soluble low-density lipoprotein receptor 11(sLR11),endothelial cell-specific molecule-1(ESM-1),and advanced glycosylation end products(AGE)in patients with preeclampsia(PE)and their ability to predict adverse pregnancy outcomes.Methods A total of 141 PE patients(PE group)and 60 normal pregnant women(control group)who were admitted to the Zhangjiakou Maternal and Child Health Hospital from January 2020 to October 2022 were selected.Serum levels of sLR11,ESM-1,and AGE were detected in each group.PE patients were divided into mild preeclampsia(MP,n=78)and severe preeclampsia(SP,n=63)according to the severity of the disease.PE patients were also divided into an adverse pregnancy outcome group(n=57)and a good pregnancy outcome group(n=84)based on the occurrence of adverse pregnancy outcomes.Receiver operating characteristic(ROC)curve analysis was used to evaluate the ability of serum levels of sLR11,ESM-1,and AGE to predict adverse pregnancy outcomes in PE patients.Pearson correlation analysis was used to examine the correlation between serum levels of sLR11,ESM-1,and AGE in PE patients.Results Serum levels of sLR11,ESM-1,and AGE were significantly higher in the PE group than in the control group(all P<0.001).Serum levels of sLR11,ESM-1,and AGE were significantly higher in the SP group than in the MP group(all P<0.001).Serum levels of sLR11,ESM-1,and AGE were significantly higher in the adverse pregnancy outcome group than in the good pregnancy outcome group(all P<0.001).ROC curve analysis showed that the combination of sLR11≥11.65μg/L,ESM-1≥2.14μg/L,and AGE≥57.38 ng/ml had the largest area under the curve(AUC)for predicting adverse pregnancy outcomes in PE patients(0.947,95%CI:0.890-0.995),with a sensitivity of 97.3%and specificity of 82.0%.Pearson correlation analysis showed that serum levels of sLR11,ESM-1,and AGE were positively correlated in PE patients(all P<0.001).Conclusions Serum levels of sLR11,ESM-1,and AGE are significantly increased in PE patients and a
