摘要
肝与大肠相通理论出自脏腑相通理论,首见于明代李梴《医学入门》.无论是从金木交合的五行气化理论去理解,还是从血能润肠、肠能导滞而助疏泄的形质层面去理解,又或是从六经开阖枢的同气相求关系去推导,都能证实肝与大肠之间的密切关系.同时,肝与大肠分别透过脏腑对应肢体以及经络循行而与肩项部有所联系.现代社会人们对手机的过度使用(久视伤血),加上熬夜作息颠倒(肝血不藏),使得临床上许多肩项疼痛由肝血不足、筋脉失养所导致.肝血不足可使肝气郁结,还可使大肠失去濡润而便秘,通过养肝血以润肠、通大肠腑以疏肝,能使此类型的肩项疼痛得到很好的缓解,这与一般临床从寒凝、气滞、血瘀等方向论治有较大的区别.肝与大肠相通理论为肩项部疼痛的治疗提供新思路.
The theory of the connection between liver and large intestine is a part of the theory of the connection of zang-fu organs.The earliest record of this theory is seen in the book Introduction to Medicine by Li Chan published in the Ming Dynasty.We can understand the close relationship between liver and large intestine in terms of the interaction between wood and metal according to the theory of five elements.Also,blood can moisten large intestine and removing stagnancy and obstruction in the intestines can help soothe the liver.In addition,according to the opening,closing and pivoting nature of the six meridians,the foot jueyin liver meridian and the hand taiyang large intestine meridian correspond with each other as both are of closing nature.In addition,liver and large intestine are associated with shoulder and neck based on the correspondence between zang-fu organs and body parts,and on meridian circulations.In the modern society,cellphone has been overused,leading to eyestrain,which in turn impairs blood as long looking at something impairs blood.Furthermore,the night-owl lifestyle negatively impacts liver’s function of storing blood.As a result,the shoulder and neck pain of many patients is caused by liver blood insufficiency and sinew and muscle malnutrition.Liver blood insufficiency gives rise to stagnation of liver qi and constipation,since the large intestine cannot be moistened.Thus,clinical practitioners can nourish liver blood to moisten large intestine and remove stagnancy and obstruction in large intestine to soothe the liver,which will have excellent curative effect on this kind of shoulder and neck pain.As this method is rather different from treatments targeted at cold congealing,qi stagnation and blood stasis,it could provide a new approach to the treatment of shoulder and neck pain.
