摘要
目的:探讨调强放疗(IMRT)联合高强度聚焦超声热疗治疗局部晚期胰腺癌的疗效及副反应。方法2008年10月~2011年9月对45例Ⅲ、Ⅳa期胰腺癌患者进行调强放疗,1次/d,5次/w,高强度聚焦超声热疗,隔日1次,共3~6次。评估肿瘤的缓解率,1年和2年生存率,临床受益率及副反应。结果总有效率(CR+PR)分别为73.3%,中位生存期14.2个月,1、2年生存率为53.3%、26.7%,毒副作用:白细胞减少、恶心呕吐。结论调强放疗联合高强度聚焦超声热疗对局部晚期胰腺癌有一定疗效,副作用少,值得进一步研究。
Objective To ef icacy and safety of intensity-modulated radiotherapy(IMRT)combined with high intensity focused ultrasound(HIFU)in treating patients with local pancreatic cancer.Methods Between October 2008 and December 2011,15patients withⅢ、Ⅳa stage pancreatic adenocarcinoma were accepted IMRT combined with HIFU.IMRT 2Gy/F,50-60Gy/25-30F/5-6W,HIFU 3~6F.The tumor remission rates,survival rates,clinical benefit response(CBP)and side ef ects were analyzed.Results The total ef ective rat(CR+PR)73.3.8%,The median survival time was 14.2 months,the 1 year,2yearsurvival rat was53.3%、26.7%.Conclusion While preliminary results indicate promising results with IMRT using HIFU and chemotherapy for local y advanced pancreatic cancer,the complication rate significantly.Reliable devived from mature phaseⅢtrials are greatly needed.
