摘要
目的:探讨格列美脲联合甘精胰岛素治疗2型糖尿病对患者胰岛β细胞功能及肾上腺皮质的影响。方法:选取2017年5月—2018年10月我院收治的2型糖尿病患者78例,以随机数字表法分为两组,各39例。对照组采用格列美脲(2 mg)联合精蛋白生物合成人胰岛素(0.15 U/kg)治疗,观察组采用甘精胰岛素(10 U)联合格列美脲(2 mg)治疗,比较两组治疗前后胰岛β细胞功能、肾上腺皮质指标及低血糖事件发生情况。结果:治疗后观察组空腹血糖(FPG)、餐后2 h血糖(2 hPG)水平均低于对照组,β细胞功能指数(△Cp/△G)高于对照组,差异有统计学意义(P <0.05);治疗后观察组尿游离皮质醇(UFC)水平低于对照组,差异有统计学意义(P <0.05);观察组低血糖事件发生率明显低于对照组,差异有统计学意义(P <0.05)。结论:格列美脲联合甘精胰岛素可有效调节2型糖尿病患者胰岛β细胞功能及肾上腺皮质,减少低血糖发生率。
Objective:To investigate the effect of glimepiride combined with insulin glargine on isletβcell functions and adrenal cortex in patients with type 2 diabetes mellitus.Methods:78 patients with type 2 diabetes mellitus admitted to our hospital from May 2017 to October 2018 were selected and randomly divided into two groups,39 cases each.The patients in the control group were treated with glimepiride(2 mg)combined with protamine biosynthetic human insulin(0.15 U/kg),and the patients in the observation group were treated with insulin glargine(10 U)plus glimepiride(2 mg).The isletβcell functions,adrenal cortex indexes and hypoglycemia events occurred before and after treatment were compared between the two groups.Results:After treatment,the levels of fasting blood glucose(FPG)and postprandial 2 h blood glucose(2 hPG)in the observation group were lower than those in the control group,and theβcell function index(△Cp/△G)was higher than that in the control group,with statistically significant differences(P<0.05).After treatment,the level of urinary free cortisol(UFC)in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).The incidence of hypoglycemia in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:Glimepiride combined with insulin glargine can effectively regulate isletβcell functions and adrenal cortex in patients with type 2 diabetes mellitus and reduce the incidence of hypoglycemia.
