摘要
目的 探究下肢力量训练对2型糖尿病周围神经病变患者下肢肌力及动态平衡功能的影响。方法 选取该院2019年7月—2020年11月收治的122例2型糖尿病周围神经病变患者,按照随机数字表法分为对照组和观察组,每组61例。两组均接受常规对症治疗,在此基础上,对照组接受常规有氧运动训练,观察组在对照组基础上接受下肢力量训练。比较两组下肢肌力、动态平衡功能、静态平衡功能、神经传导功能及血糖指标。结果 干预前,两组下肢肌力测试动作次数、动态平衡功能、静态平衡功能、神经传导功能及各项血糖指标水平比较,组间差异无统计学意义(P>0.05);干预后,观察组下肢肌力测试动作次数多于对照组,动态平衡功能测试时间短于对照组,睁眼及闭眼单足站立时间均长于对照组,腓总神经感觉神经传导速度、腓总神经运动神经传导速度、远端胫后神经感觉神经传导速度、近端胫后神经感觉神经传导速度均快于对照组,空腹血糖、餐后2 h血糖、糖化血红蛋白水平及胰岛素抵抗指数均低于对照组,组间差异有统计学意义(P<0.05)。结论 下肢力量训练能够有效改善2型糖尿病周围神经病变患者下肢肌力,提高其静态及动态平衡功能,控制血糖水平,促进神经功能恢复。
Objective To explore the effect of lower extremity strength training on lower extremity muscle strength and dynamic balance function in patients with type 2 diabetic peripheral neuropathy.Methods A total of 122 patients with type 2 diabetic peripheral neuropathy who were admitted to our hospital from July 2019 to November 2020 were selected and divided into a control group and an observation group according to the random number table method,with 61 cases in each group.Both groups received conventional symptomatic treatment,on this basis,the control group received conventional aerobic exercise training,and the observation group received lower limb strength training on the basis of the control group.The lower limb muscle strength,dynamic balance function,static balance function,nerve conduction function and blood glucose indexes were compared between the two groups.Results Before the intervention,the number of lower limb muscle strength test,dynamic balance function,static balance function,nerve conduction function and the levels of various blood glucose indexes were compared between the two groups,and the differences between the groups were not statistically significant(P>0.05).After the intervention,the number of lower limb muscle strength test in the observation group was more than that in the control group,and the test time for dynamic balance function was shorter than that in the control group,the time of standing on one foot with eyes open and eyes closed was longer than those in the control group,the sensory nerve conduction velocity of the common peroneal nerve,the motor nerve conduction velocity of the common peroneal nerve,the sensory nerve conduction velocity of the distal posterior tibial nerve,and the sensory nerve conduction velocity of the proximal posterior tibial nerve were all faster than those of the control group,the levels of fasting blood glucose,2 h postprandial blood glucose,glycated hemoglobin|glycosylated hemoglobin and homeostasis model assessment-insulin resistance were lower than those
