单侧髋关节置换术后患者进行平衡功能训练对下肢运动功能的影响 被引量:1

Effect of Balance Function Training on Lower Extremity Motor Function after Unilateral Hip Arthroplasty

摘要 目的分析平衡功能训练对单侧髋关节置换术后患者下肢运动功能的影响.方法选取2019年6月—2021年6月本院收治的40例单侧髋关节置换术后患者为研究对象,根据随机数字表法将其分为对照组20例及研究组20例.对照组进行常规康复训练,包括关节活动度、肌力、负重、转移、步态训练等,在此基础上,研究组进行平衡功能训练,包括平衡板训练、平衡仪训练及"一字型"步行训练,两组均持续训练6周.比较两组的干预效果.结果两组干预2周的功能性步行量表(FAC)分级比较,差异无统计学意义(P>0.05);干预4、6周,研究组的FAC分级均优于对照组,组间差异有统计学意义(P<0.05).干预前,两组的最大步行速度(MWS)、Berg平衡量表(BBS)评分及跌倒指数(FI)比较,组间差异无统计学意义(P>0.05);干预2、4、6周后,研究组的MWS用时均短于对照组,BBS评分均高于对照组,FI均低于对照组,组间差异有统计学意义(P<0.05).结论单侧髋关节置换术后患者在常规训练基础上进行平衡功能训练,可改善其步行能力及机体平衡功能,值得进一步推广使用. Objective To analyze the effect of balance function training on lower limb motor function after unilateral hip arthroplasty.Methods A total of 40 patients after unilateral hip arthroplasty admitted to the Department of Bone and Joint Rehabilitation of our hospital from June 2019 to June 2021 were selected as the research objects,and they were divided into a control group(n=20)and a study group(n=20)according to random number table method.The control group received routine rehabilitation training,including range of motion,muscle strength,weight bearing,transfer,gait training,etc.On this basis,the research group received balance function training,including balance board training,balance instrument training and straight line walking training,both groups continued training for 6 weeks.The effects of intervention were compared between the two groups.Results After 2 weeks of intervention,there was no significant difference in functional ambulation category scale(FAC)grade between the two groups(P>0.05);after 4 and 6 weeks of intervention,the FAC grades of the study group were better than those of the control group,and the differences between the groups were statistically significant(P<0.05).Before intervention,there were no significant differences in maximum walking speed(MWS),Berg balance scale(BBS)score and fall index(FI)between the two groups(P>0.05);after 2,4 and 6 weeks of intervention,the MWS time of the study group were shorter than those of the control group,BBS score were higher than those of the control group,and FI were lower than those of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion After unilateral hip arthroplasty,balance function training on the basis of routine training can improve walking ability and body balance function,which is worthy of further popularization.
