摘要
目的探讨达格列净联合胰岛素在2型糖尿病(T2DM)患者中的应用效果.方法选取2020年2月—2021年4月本院收治的T2DM患者78例,按随机数字表法分为两组,每组39例.对照组采用胰岛素治疗,观察组在此基础上联合达格列净治疗,均持续治疗12周.对比两组的临床疗效、血糖控制效果、血管内皮功能及不良反应发生情况.结果观察组的临床总有效率为94.87%,高于对照组的79.49%,差异有统计学意义(P<0.05).观察组治疗后的空腹血糖、餐后2 h血糖、糖化血红蛋白水平均低于对照组,组间差异有统计学意义(P<0.05).观察组治疗后的一氧化氮、血管内皮细胞依赖性舒张功能水平均高于对照组,内皮素水平低于对照组,组间差异有统计学意义(P<0.05).两组的不良反应发生率比较,差异无统计学意义(P>0.05).结论达格列净联合胰岛素在T2DM患者治疗中具有较好的疗效,可改善血糖水平与血管内皮功能,且应用安全性较好,值得临床推广使用.
Objective To investigate the effect of dapagliflozin combined with insulin in patients with type 2 diabetes mellitus(T2DM).Methods A total of 78 T2DM patients admitted to our hospital from February 2020 to April 2021 were selected and were divided into two groups according to random number table method,with 39 patients in each group.The control group was treated with insulin,and the observation group was treated with dapagliflozin on this basis.Both groups were treated for 12 weeks.The clinical efficacy,blood glucose control effect,vascular endothelial function and the occurrence of adverse reactions were compared between the two groups.Results The total clinical effective rate of the observation group was 94.87%,higher than 79.49%of the control group,the difference was statistically significant(P<0.05).The levels of fasting blood glucose,2 h postprandial blood glucose and glycated hemoglobin A1c in the observation group were lower than those in the control group after treatment,and the differences between the groups were statistically significant(P<0.05).After treatment,the levels of nitric oxide and endothelium-dependent flow-mediated dilation in the observation group were higher than those in the control group,while the level of endothelin was lower than that in the control group,the differences between the groups were statistically significant(P<0.05).There was no statistical significance in the incidence of adverse reactions between the two groups(P<0.05).Conclusion Dapagliflozin combined with insulin has good efficacy in the treatment of T2DM patients,can improve blood glucose level and vascular endothelial function,and the safety of application is good,worthy of clinical application.
