正中神经电刺激联合镜像神经元康复训练治疗重型颅脑损伤患者的康复效果

Rehabilitation Effect of Median Nerve Electrical Stimulation Combined with Mirror Neuron Rehabilitation Training on Patients with Severe Craniocerebral Injury

摘要 目的探讨正中神经电刺激联合镜像神经元康复训练治疗重型颅脑损伤的临床效果.方法选取2020年1月—2022年1月本院重症加强护理病房收治的64例重型颅脑损伤患者,按随机数字表法分为两组,每组32例.对照组进行镜像神经元康复训练,观察组在此基础上采用正中神经电刺激治疗,均持续治疗8周.比较两组的临床疗效、格拉斯哥昏迷量表(GCS)评分、残疾分级量表(DRS)评分及脑血流动力学参数.结果观察组的治疗总有效率为93.75%,高于对照组的75.00%,差异有统计学意义(P<0.05).治疗后,观察组的GCS评分高于对照组,DRS评分低于对照组,组间差异有统计学意义(P<0.05).治疗后,观察组的大脑中动脉平均血流速度、脑血流量、脑血容量均优于对照组,组间差异有统计学意义(P<0.05).结论正中神经电刺激联合镜像神经元康复训练可提高重型颅脑损伤患者康复效果,能改善脑血流动力学参数及意识状态,利于预后. Objective To investigate the clinical effect of median nerve electrical stimulation combined with mirror neuron rehabilitation training on severe brain injury.Methods A total of 64 patients with severe craniocerebral injury admitted to intensive care unit of our hospital from January 2020 to January 2022 were selected and were divided into two groups according to the random number table method,with 32 cases in each group.The control group received mirror neuron rehabilitation training,and the observation group received median nerve electrical stimulation on this basis.Both groups were treated for 8 weeks.The clinical efficacy,Glasgow coma scale(GCS)score,disability rating scale(DRS)score and cerebral hemodynamic parameters were compared between the two groups.Results The total effective rate of the observation group was 93.75%,higher than 75.00%of the control group,and the difference was statistically significant(P<0.05).After treatment,the GCS score of the observation group was higher than that of the control group,and the DRS score was lower than that of the control group,and the differences between the groups were statistically significant(P<0.05).After treatment,the mean blood flow velocity of middle cerebral artery,cerebral blood flow and cerebral blood volume in the observation group were better than those in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Median nerve stimulation combined with mirror neuron rehabilitation training can improve the rehabilitation effect of patients with severe craniocerebral injury,improve cerebral hemodynamic parameters and consciousness,and is beneficial to prognosis.
