骨水泥强化型股骨近端防旋髓内钉治疗老年重度骨质疏松性股骨转子间骨折的临床疗效 被引量:3

摘要 目的探讨老年重度骨质疏松性股骨转子间骨折患者选择骨水泥强化型股骨近端防旋髓内钉(PFNA)的疗效及安全性.方法选择2018年1月-2021年11月于本院接受手术治疗的63例老年重度骨质疏松性股骨转子间骨折患者为研究对象,按照入院时患者病案尾号奇偶数分为两组,观察组为奇数编号,对照组为偶数编号.对照组(n=31)采用PFNA治疗,观察组(n=32)采用骨水泥强化型PFNA治疗.对比两组患者手术指标、髋关节各方向活动角度及并发症发生情况.结果观察组患者术后下床、负重练习及骨折完全愈合时间均短于对照组,组间差异有统计学意义(P<0.05).治疗前,两组患者髋关节各方向活动角度比较,组间差异无统计学意义(P>0.05);治疗后,观察组患者髋关节内收、外展、屈曲、外旋活动角度均大于对照组,组间差异有统计学意义(P<0.05).观察组术后出现并发症发生率为3.13%,低于对照组的25.81%,差异有统计学意义(P<0.05).结论临床治疗老年重度骨质疏松性股骨转子间骨折可首选骨水泥强化型PFNA术式,能使其更早进行负重活动,恢复髋关节功能,且临床并发症少,值得推广应用. Objective To investigate the efficacy and safety of bone cement-enhanced proximal femoral nail antirotation(PFNA)in senile patients with severe osteoporotic femoral intertrochanteric fractures.Methods A total of 63 senile patients with severe osteoporotic femoral intertrochanteric fractures who received surgical treatment in our hospital from January 2018 to November 2021 were selected as the research objects,and they were divided into two groups according to the odd and even numbers of the patients'medical records at the time of admission.The observation group is odd-numbered,and the control group is even-numbered.The control group(n=31)was treated with PFNA,and the observation group(n=32)was treated with bone cement-enhanced PFNA.The surgical indexes,hip joint movement angles in all directions and the incidence of complications were compared between the two groups.Results The time of getting out of bed,weight-bearing exercise and complete fracture healing time in the observation group was shorter than that in the control group,and the differences between the groups were statistically significant(P<0.05).Before treatment,there were no significant differences between the two groups in the hip joint movement angles in all directions(P>0.05);after treatment,the hip joint adduction,abduction,flexion and external rotation of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).The incidence of postoperative complications in the observation group was 3.13%,which was lower than 25.81%in the control group,and the differences were statistically significant(P<0.05).Conclusion Bone cement-enhanced PFNA is the first choice for the clinical treatment of severe osteoporotic intertrochanteric fractures in the senile,which can enable earlier weight-bearing activities and restore hip function with fewer clinical complications,which is worthy of popularization and application.
