Effect of Granulocyte Colony-Stimulating Factor Combined with Half-Dose Plasma Exchange and Dual Plasma Molecular Adsorption System on Liver Function and the Levels of Serum Inflammatory Factors in Patients with Liver Failure

摘要 目的分析粒细胞集落刺激因子(G-CSF)联合半量血浆置换(HPE)与双重血浆分子吸附系统(DPMAS)在肝衰竭患者中的应用效果.方法选取2020年4月—2022年4月该院诊治的60例肝衰竭患者为研究对象,按随机数字表法分为两组,各30例.对照组采用HPE与DPMAS,观察组在对照组基础上加用G-CSF,均共治疗7 d.对比两组肝功能、炎症因子水平、临床疗效.结果治疗前,两组总胆红素(TBIL)、天冬氨酸转氨酶(AST)、丙氨酸转氨酶(ALT)、白细胞介素-6(IL-6)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)水平相比,组间差异无统计学意义(P>0.05);治疗后,观察组TBIL、AST、ALT、IL-6、CRP、TNF-α水平均低于对照组,组间差异有统计学意义(P<0.05).观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05).结论G-CSF联合HPE与DPMAS治疗肝衰竭患者的效果显著,可有效改善患者的肝功能,降低炎症因子水平,值得临床推广. Objective To analyze granulocyte colony-stimulating factor(G-CSF)combined with half-dose plasma exchange(HPE)and dual plasma molecular absorption system(DPMAS)in patients with liver failure.Methods 60 patients with liver failure diagnosed and treated in the hospital from April 2020 to April 2022 were selected as the research objects and divided into two groups according to the random number table method,with 30 cases in each group.The control group was treated with HPE and DPMAS,while the observation group was treated with G-CSF on the basis of the control group.Both groups were treated for 7 days.The liver function,levels of inflammatory factors and clinical efficacy were compared between the two groups.Results Before treatment,there were no significant differences in the levels of total bilirubin(TBIL),aspartate aminotransferase(AST),alanine aminotransferase(ALT),interleukin-6(IL-6),C-reactive protein(CRP),and tumor necrosis factor-α(TNF-α)between the two groups(P>0.05);after treatment,the levels of TBIL,AST,ALT,IL-6,CRP and TNF-αin the observation group were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion G-CSF combined with HPE and DPMAS has a significant effect in the treatment of patients with liver failure,which can effectively improve the liver function of patients and reduce the level of inflammatory factors,which is worthy of clinical promotion.
