生物反馈电刺激联合阴道锥体训练对更年期压力性尿失禁患者盆底肌力及排尿情况的影响

Effect of Biofeedback Electrical Stimulation Combined with Vaginal Cone Training on Pelvic Floor Muscle Strength and Urination in Patients with Menopausal Stress Urinary Incontinence

摘要 目的探讨生物反馈电刺激联合阴道锥体训练对更年期压力性尿失禁患者盆底肌力及排尿情况的影响.方法选择2020年5月—2022年2月两院收治的90例更年期压力性尿失禁患者为研究对象,按随机数字表法分为对照组(n=45)和观察组(n=45).对照组采用常规盆底肌力训练,观察组在对照组基础上采用生物反馈电刺激联合阴道锥体训练,两组均持续干预6个月.对比两组盆底肌力、排尿及尿失禁情况.结果两组干预前盆底肌力、排尿及尿失禁情况比较,组间差异无统计学意义(P>0.05);观察组干预后Ⅰ类肌纤维肌电值、Ⅱ类肌纤维肌电值均高于对照组,每日排尿次数、每日漏尿次数、1h尿垫试验漏尿量均少于对照组,最大排尿量多于对照组,国际尿失禁咨询委员会尿失禁问卷简表中漏尿量、漏尿频率、疾病对生活的影响评分及总分均低于对照组,组间差异有统计学意义(P<0.05).结论对更年期压力性尿失禁患者采取生物反馈电刺激联合阴道锥体训练进行干预,能够明显增强患者的盆底肌力,改善排尿及尿失禁情况,有较高的临床应用价值. Objective To investigate the effect of biofeedback electrical stimulation combined with vaginal cone training on pelvic floor muscle strength and urination in patients with menopausal stress urinary incontinence.Methods 90 patients with menopausal stress urinary incontinence admitted to both hospitals from May 2020 to February 2022 were selected as the research objects.All patients were divided into a control group(n=45)and an observation group(n=45)according to random number table method.The control group used routine pelvic floor muscle strength training,and the observation group used biofeedback electrical stimulation combined with vaginal cone training on the basis of the control group.Both groups continued to intervene for 6 months.The pelvic floor muscle strength,urination and urinary incontinence were compared between the two groups.Results There was no significant difference in pelvic floor muscle strength,urination and urinary incontinence between the two groups before intervention(P>0.05);after the intervention,the electromyography value of classⅠmuscle fibers and classⅡmuscle fibers of the observation group were higher than those of the control group,the number of urination per day,the number of urine leakage per day,and the amount of urine leakage in the 1 h urine pad test were less than those of the control group,and the maximum amount of urine excretion was more than that of the control group,the scores of amount of urine leakage,the frequency of urine leakage,and the impact of diseases on life and total scores in International Consultation on Incontinence questionnaire-short form were lower than those of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion The intervention of biofeedback electrical stimulation combined with vaginal cone training in patients with menopausal stress urinary incontinence can significantly enhance the pelvic floor muscle strength and improve urination and urinary incontinence,which has high clinical application v
