摘要
目的探究本体感觉训练联合持续被动活动对髌骨骨折内固定术后患者膝关节活动度的影响.方法选取2021年1月—2022年7月于我院行髌骨骨折内固定术的患者102例为研究对象,按随机数字表法分为对照组(n=51)及观察组(n=51),术后,对照组进行常规康复训练,观察组进行本体感觉训练联合持续被动活动,均持续3个月.对比两组患者的关节活动度、膝关节功能、疼痛程度、平衡功能及生活质量.结果干预后,观察组的膝关节活动度为(125.35±6.22)°,大于对照组的(115.06±7.09)°,差异有统计学意义(P<0.05).干预后,观察组的膝关节功能评分、Berg平衡量表评分、生活质量综合评定问卷评分分别为(84.21±5.01)分、(44.13±4.19)分、(80.15±4.52)分,均高于对照组的(78.14±6.28)分、(37.71±3.44)分、(72.34±4.67)分,组间差异有统计学意义(P<0.05).干预后,观察组的疼痛视觉模拟评分为(0.89±0.53)分,低于对照组的(1.21±0.79)分,差异有统计学意义(P<0.05).结论本体感觉训练联合持续被动活动在髌骨骨折内固定术后患者中的应用效果理想,可促进疼痛缓解,改善膝关节功能及平衡功能,提升膝关节活动度及生活质量.
Objective To explore the effect of proprioception training combined with continuous passive movement on knee range of motion after internal fixation of patellar fracture.Methods A total of 102 patients who received internal fixation of patellar fracture in our hospital from January 2021 to July 2022 were selected as the research objects and were divided into a control group(n=51)and an observation group(n=51)according to random number table method.After surgery,the control group received routine rehabilitation training,and the observation group received proprioception training combined with continuous passive movement.Both groups lasted for 3 months.The range of motion,knee function,degree of pain,balance function and quality of life were compared between the two groups.Results After intervention,the knee range of motion in the observation group was(125.35±6.22)°,greater than(115.06±7.09)°in the control group,the difference was statistically significant(P<0.05).After intervention,the knee function score,Berg balance scale score and generic quality of life inventory-74 score of the observation group were(84.21±5.01)points,(44.13±4.19)points and(80.15±4.52)points,respectively,which were higher than(78.14±6.28)points,(37.71±3.44)points and(72.34±4.67)points in the control group,and the differences between the groups were statistically significant(P<0.05).After intervention,the visual simulation scale score of pain in the observation group was(0.89±0.53)points,lower than(1.21±0.79)points in the control group,the difference was statistically significant(P<0.05).Conclusion Proprioception training combined with continuous passive movement is ideal in patients after internal fixation of patellar fracture.It can promote pain relief,improve knee function and balance function,and improve knee motion and quality of life.
