俯卧位机械通气干预对低氧血症患者血流动力学及氧合指数的影响

Effect of Mechanical Ventilation Intervention in Prone Position on Hemodynamics and Oxygenation Index in Patients with Hypoxemia

摘要 目的分析俯卧位机械通气在低氧血症患者中的应用效果。方法选取2021年2月—2023年2月广州医科大学附属第一医院收治的92例低氧血症患者为研究对象,按随机数字表法分为对照组及观察组,每组46例。对照组行常规仰卧位机械通气,观察组行俯卧位机械通气。对比两组患者的血流动力学、氧合指数及并发症发生情况。结果干预3 d后,观察组的中心静脉压为(3.21±0.23)cmH_(2)O(1 cmH_(2)O=0.098 kPa),低于对照组的(3.96±0.54)cmH_(2)O,氧合指数、全身血管阻力分别为(341.54±30.53)mmHg(1 mmHg=0.133 kPa)、(12.28±2.69)%,均高于对照组的(297.83±26.97)mmHg、(10.54±2.01)%,组间差异有统计学意义(P<0.05)。观察组的并发症发生率为4.35%,低于对照组的17.39%,差异有统计学意义(P<0.05)。结论相较于仰卧位机械通气,俯卧位机械通气应用于低氧血症患者中的效果更佳,能改善其血流动力学,提高氧合指数,且并发症少,具有较高的应用价值。 Objective To analyze the effect of prone mechanical ventilation in patients with hypoxemia.Methods 92 patients with hypoxemia admitted to the First Affiliated Hospital of Guangzhou Medical University from February 2021 to February 2023 were selected as the study objects and were divided into a control group and an observation group according to random number table method,with 46 cases in each group.The control group received mechanical ventilation in supine position,and the observation group received mechanical ventilation in prone position.The hemodynamics,oxygenation index and complications were compared between the two groups.Results After 3 days of intervention,the central venous pressure in the observation group was(3.21±0.23)cm H_(2)O(1 cm H_(2)O=0.098 kPa),lower than(3.96±0.54)cm H_(2)O in the control group,while the oxygenation index and systemic vascular resistance index were(341.54±30.53)mmHg(1 mmHg=0.133 kPa)and(12.28±2.69)%,respectively,which were higher than(297.83±26.97)mm Hg and(10.54±2.01)%in the control group,and the differences between the groups were statistically significant(P<0.05).The incidence of complications in the observation group was 4.35%,which was lower than 17.39%in the control group,and the difference was statistically significant(P<0.05).Conclusion Compared with supine mechanical ventilation,prone mechanical ventilation has better effect in patients with hypoxemia,can improve hemodynamics,increase oxygenation index,has fewer complications,and has higher application value.
