摘要
目的探讨早期与延期交叉韧带重建手术分别联合加速康复外科理念护理在急性前交叉韧带损伤(ACL)患者中的应用效果。方法选取2022年1—11月间南京市第一医院收治的94例急性ACL患者为研究对象,根据接受手术的时机不同分为观察组与对照组,每组47例。观察组于受伤6周内接受手术,对照组于受伤6周后接受手术,两组围术期均接受加速康复外科理念护理,术后均随访6个月。比较两组患者的护理效果。结果术后6个月,观察组的膝关节主观评分、膝关节Lysholm评估量表评分、Berg平衡量表评分均高于对照组,疼痛视觉模拟评分低于对照组,组间差异有统计学意义(P<0.05)。术后6个月,观察组的膝关节活动度均大于对照组,屈膝受限度数、伸膝受限度数、大腿周径差值均小于对照组,组间差异有统计学意义(P<0.05)。术后6个月,观察组的胫骨前移距离小于对照组,Lachman试验阳性、前抽屉试验阳性患者占比均低于对照组,组间差异有统计学意义(P<0.05)。随访期间,观察组的并发症总发生率为4.26%,低于对照组的19.15%,差异有统计学意义(P<0.05)。结论早期交叉韧带重建手术联合加速康复外科理念护理可有效促进急性ACL患者膝关节功能恢复,减轻肌肉萎缩程度,减少术后并发症,改善预后。
Objective To investigate the effect of early and delayed cruciate ligament reconstruction combined with enh-anced recovery after surgery concept nursing in patients with acute anterior cruciate ligament injury(ACL).Methods 94 patients with acute ACL admitted to the Nanjing First Hospital from January to November 2022 were selected as the study objects and were divided into an observation group and a control group according to the time of operation,with 47 cases in each group.The observation group received surgery within 6 weeks after injury,and the control group received surgery 6 weeks after injury.Both groups received enhanced recovery after surgery concept nursing during perioperative period,and were followed up for 6 months.The nursing effects of the two groups was compared.Results 6 months after surgery,the international knee documentation committee knee evaluation form,knee joint Lysholm evaluation scale score and Berg Balance Scale score of the observation group were higher than those of the control group,and the pain Visual Analogue Scale score was lower than that of the control group,the differences between the groups were statistically significant(P<0.05).6 months after surgery,the knee motion of the observation group was greater than that of the control group,and the knee bending limitation degree,knee extension limitation degree and the difference of thigh circumference diameter were smaller than those of the control group,the differences between the groups were statistically significant(P<0.05).6 months after surgery,the tibial anterior distance of the observation group was smaller than that of the control group,and the proportions of patients with positive Lachman test and positive front drawer test were lower than those of the control group,the differences between the groups were statistically significant(P<0.05).During the follow-up period,the total incidence of complications in the observation group was 4.26%,which was lower than 19.15%in the control group,and the difference was statistically sig
