脑深部电极植入术对帕金森病患者冻结步态及日常活动能力的影响

Effect of Deep Brain Electrode Implantation on Freezing Gait and Activities of Daily Living in Patients with Parkinson's Disease

摘要 目的探讨脑深部电极植入术(DBS)对帕金森病(PD)患者冻结步态及日常活动能力的影响.方法选择2021年6月—2023年3月该院收治的50例PD患者作为研究对象,根据随机数字表法将其分为对照组和观察组,各25例.对照组行常规治疗,观察组加用DBS治疗.比较两组患者治疗后冻结步态﹑日常生活活动能力量表(ADL)评分﹑步长﹑步速.结果治疗前,两组冻结步态﹑ADL评分﹑步长﹑步速对比,组间差异无统计学意义(P>0.05).治疗后,观察组冻结步态评分为(11.65±2.33)分,低于对照组的(16.27±4.76),ADL评分为(85.69±8.45)分,高于对照组的(72.19±7.71)分,组间差异有统计学意义(P<0.05).治疗后,观察组步长长于对照组,步速快于对照组,组间差异有统计学意义(P<0.05).结论PD患者采用DBS治疗效果确切,利于改善患者冻结步态,提升日常生活活动能力,增加患者步幅,加快步速,值得临床广泛应用. Objective To investigate the effect of deep brain electrode implantation(DBS)on frozen gait and daily activity ability in patients with Parkinson's disease(PD).Methods 50 PD patients admitted to the hospital from June 2021 to March 2023 were selected as an research objects.According to the random number table method,they were divided into a control group and an observation group,with 25 cases in each group.The control group was treated with routine treatment,and the observation group was treated with DBS.The freezing of gait,activities of daily living scale(ADL)score,step length and step speed were compared between the two groups after treatment.Results Before treatment,there was no significant difference in freezing gait,ADL score,step length and step speed between the two groups(P>0.05).After treatment,the freezing of gait score of the observation group was(11.65±2.33)points,which was lower than(16.27±4.76)points of the control group,and the ADL score was(85.69±8.45)points,which was higher than(72.19±7.71)points of the control group,the differences between the two groups were statistically significant(P<0.05).After treatment,the step length of the observation group was longer than that of the control group,and the step speed was faster than that of the control group,and the differences between the two groups were statistically significant(P<0.05).Conclusion DBS is effective in the treatment of PD patients,which is conducive to improving the freezing of gait,improving the ability of activities of daily living,increasing the stride length of patients,and accelerating the step speed,which is worthy of clinical application.
