摘要
目的探究利妥昔单抗联合LED红光治疗仪、激素和银离子敷料在中重度天疱疮患者中的应用效果。方法选取2015年1月—2021年6月我院收治的60例中重度天疱疮患者为研究对象,采用随机数字表法将其分为对照组(n=30)、观察组(n=30)。对照组采用LED红光治疗仪、激素和银离子敷料治疗,观察组在对照组治疗措施基础上联合应用利妥昔单抗治疗。比较两组愈合时间、治疗前后抗大疱性类天疱疮(BP)180抗体、白细胞介素6(IL-6)、肿瘤坏死因子-α(TNF-α)水平,以及治疗后3、6、12个月复发率。结果观察组皮损愈合时间、炎症消退时间、水疱消失时间均短于对照组,组间差异有统计学意义(P<0.05)。治疗后,两组抗BP180抗体、TNF-α、IL-6水平均较治疗前下降,且观察组低于对照组,差异有统计学意义(P<0.05)。治疗后6、12个月,观察组复发率低于对照组,组间差异有统计学意义(P<0.05)。结论皮损清创后使用利妥昔单抗联合LED红光治疗仪、激素和银离子敷料治疗中重度天疱疮患者具有较高的应用价值。
Objective To explore the application effect of rituximab combined with LED red light therapy instrument,hormone and silver ion dressing in patients with moderate to severe pemphigus.Methods From January 2015 to June 2021,60 patients with moderate to severe pemphigus were selected as the study objects and divided into a control group(n=30)and an observation group(n=30)by random number table method.The control group was treated with LED red light therapy instrument,hormone and silver ion dressing.The observation group was treated with rituximab on the basis of the above.The healing time,the levels of BP180 antibody,IL-6 and TNF-α before and after treatment and the recurrence rate were compared between the two groups.Results The healing time of skin lesions,inflammation resolution time,and blister disappearance time in the observation group were shorter than those in the control group,and the differences between the groups were statistically significant(P<0.05).After treatment,the levels of anti-BP180 antibody,TNF-α,and IL-6 in both groups decreased compared to before treatment,and the levels of anti-BP180 antibody,TNF-α,and IL-6 in the observation group were lower than those in the control group(P<0.05).At 6 and 12 months after treatment,the recurrence rate of the observation group were lower than those of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion The use of rituximab combined with LED red light therapy instrument,hormone and silver ion dressing after debridement of skin lesions has high application value.
