摘要
目的探讨多沙唑嗪、中药癃闭舒胶囊联合膀胱经皮电刺激治疗膀胱老化的疗效.方法经尿动力学检查提示逼尿肌收缩无力和(或)逼尿肌不稳定收缩的下尿路症状患者175例,随机分为多沙唑嗪组(n=25)、中药癃闭舒胶囊组(n=25)、膀胱电刺激组(n=25)、多沙唑嗪联合癃闭舒组(n=25)、多沙唑嗪联合膀胱电刺激组(n=25)、癃闭舒联合膀胱电刺激组(n=25)、多沙唑嗪/癃闭舒联合膀胱经皮电刺激组(n=25),治疗前、治疗后4周、治疗后8周分别进行随访.以国际前列腺症状评分(International Prostate Symptom Score,IPSS)、生活质量(quality of life,QoL)评分、最大尿流率(maximum urinary flow rate,Qmax)和残余尿(post-void residual volume,PVR)为主要指标评价其疗效.结果各治疗组之间患者年龄、治疗前的Qmax、PVR无显著差异(P>0.05).单用多沙唑嗪有效率28%,单用癃闭舒胶囊有效率24%,单用膀胱电刺激有效率36%,多沙唑嗪联合癃闭舒治疗有效率56%,多沙唑嗪联合膀胱电刺激治疗有效率64%,癃闭舒联合膀胱电刺激治疗有效率56%,多沙唑嗪,癃闭舒联合膀胱经皮电刺激治疗有效率60%.综合疗效优于单药治疗组(P<0.05);各组治疗后平均IPSS和QoL评分均有显著改善,各组治疗后Qmax均显著提高(P<0.05).结论对尿动力学证实为膀胱老化的下尿路症状患者,多沙唑嗪、癃闭舒胶囊及膀胱电刺激均有良好疗效,各联合治疗方案的疗效优于各单治疗方案,联合治疗方案组间并无疗效差异,对于膀胱老化患者建议使用至少两种治疗方案联合.
Objective To investigate the therapeutic effects of doxazosin,Longbishu capsule combined with bladder transcutaneous electrical stimulation on aging bladder.Methods One hundred and seventy-five patients with lower urinary tract symptoms suggestive of weak and/or unstable contraction of the urinary muscle by urodynamic examination were randomly divided into doxazosin group(n=25),traditional Chinese medicine Longbishu capsule group(n=25),bladder electrical stimulation group(n=25),doxazosin combined with Longbishu group(n=25),doxazosin combined with bladder electrical stimulation group(n=25),Longbishu combined with bladder electrical stimulation group(n=25),and doxazosin/Longbishu combined with bladder transcutaneous electrical stimulation group(n=25),and were followed up before the treatment,4 weeks after treatment,and 8 weeks after treatment.The International Prostate Symptom Score(IPSS),quality of life(QoL)score,maximum urinary flow rate(Qmax)and residual urine(PVR)were used as the main indicators to evaluate the efficacy.Results There was no significant difference in age,Qmax and PVR before the treatment among the treatment groups(P>0.05).The effective rate of Doxazosin group was 28%,the effective rate of Longbishu Capsule group was 24%,the effective rate of bladder electrical stimulation group was 36%,the effective rate of Doxazosin combined with Longbishu group was 56%,the effective rate of Doxazosin combined with bladder electrical stimulation group was 64%,the effective rate of Longbishu combined with bladder electrical stimulation group was 56%,and the effective rate of Doxazosin,Longbishu combined with bladder transcutaneous electrical stimulation group was 60%.The comprehensive curative effect was better than that of the treatment group(P<0.05);the mean IPSS and QoL scores were significantly improved after treatments in each group,and the Qmax was significantly increased after treatment in each group(P<0.05).Conclusions The Doxazosin,Longbishu capsule and bladder electrical stimulation are effective for t
