摘要
目的:调查先天性肛门直肠畸形(ARM)术后延续性护理需求情况,并分析其影响因素。方法:选取2019年4月至2022年4月我院接诊并接受手术治疗的40例先天性肛门直肠畸形患儿作为研究对象,通过自制调查问卷对患儿主要照护者进行调查,分析其延续性护理需求,并应用单因素分析和多元逐步回归分析法分析护理需求的影响因素。结果:肠道综合管理需求、肛门功能康复促进需求和疾病健康管理需求评分分别为(4.14±0.42)分、(4.06±0.45)分和(4.01±0.42)分,平均(4.06±0.67)分。多元逐步回归分析显示,疾病分型(中高位无肛)、手术方式(肛门成形术)、主要照护者文化程度是术后肠道综合管理需求的影响因素;疾病分型、主要照护者文化程度、患儿父母职业是术后肛门功能康复促进需求的影响因素;疾病分型、手术方式、主要照护者文化程度、患儿父母职业是术后疾病健康管理需求的影响因素。结论:ARM术后延续性护理需求受多因素影响,医护工作者在制定延续性护理计划时,应以照护者需求为导向,制定有效的延续性护理方案,提高护理质量,进而提高患儿生活质量。
Objective To investigate the continuity nursing demand(CND)status after the surgery for congenital anorectal malformations(ARM),and to analyse its influence factors.Methods Enrolled 40pediatric patients with congenital ARM diagnosed and surgically treated in authors'hospital(2019-04-2022-04)into this research:investigated their main caring persons by using self-developed questionnare,and analysed the CND,then,by using Uni-variate analysis and Multi-variate regression analysis method analysed the factors influencing CND.Results The ratings on the comprehensive management demand(CMD)for intestine tract,the anal function promotion demand(AFPD),and health management deman(HMD)for disease were respectively(4.14±0.42)points,(4.06±0.45)points,and(4.01±0.42)points,which their averaged points were(4.06±0.67).Multi-variate regression analysis showed the factors influencing postoperative CMD for intestine tract were disease's typies(mid-,high site,and no-anus),procedures(anoplasty),the educated levels of main caring persons;the factors influencing postoperative AFPD were disease's typies,the educated levels of main caring persons,and patients'parents'occupations;the postoperative HMD influencing factors were diseases'typies,procedures,the educated levels of main caring persons,and patients'parents'occupations.Conclusion CND after the procedure for ARM are influenced by many factors,therefore,when developing CN plan,it should be guided by caring persons'demands,making CN project effective,enhancing nursing quality,further,pediatric patients'quality of life.
