喉返神经入喉处解剖暴露在甲状腺腺叶切除术中的应用价值

摘要 目的:研究喉返神经(RLN)入喉处解剖暴露在甲状腺腺叶切除术(TTD)中的应用价值及安全性。方法:选取86例甲状腺乳头状癌(PTC)患者,按随机数字表法将其分为对照组和观察组,每组各43例。两组均行TTD治疗,对照组采用甲状腺下动脉下方解剖暴露处理RLN,观察组采用RLN入喉处解剖暴露处理。比较两组手术时间、术中出血量、疼痛程度、声带功能及并发症。结果:观察组手术时间短于对照组,术中出血量及视觉模拟评分法(VAS)评分低于对照组,术后基频微扰及振幅微扰低于对照组,并发症总发生率低于对照组,两组比较差异均具有统计学意义(P<0.05)。结论:TTD术中入喉处解剖暴露处理RLN具有损伤小、并发症发生率低的优点,可减轻对患者声带功能的影响。 Objective:To study the application value and safety of the anatomical exposure of the recurrent laryngeal nerve(RLN)entering the larynx in thyroid lobectomy(TTD).Methods:86 patients with PTC were selected and randomized into the control group and observation group,with 43 cases in each group.Both groups were treated with TTD,RLN of the control group were treated with anatomical exposure below the inferior thyroid artery,and the observation group were treated with anatomical exposure where RLN enters the larynx.The operation time,intraoperative blood loss,pain degree,vocal cord function and complications were compared between the two groups.Results:The operation time of the observation group was shorter than that of the control group,the intraoperative blood loss and visual analogue scale(VAS)score were lower than those of the control group,and the postoperative jitter and shimmer were lower than those of the control group and the total incidence of complications was lower than that of the control group,and the differences were statistically significant between both groups(P<0.05).Conclusion:The anatomical exposure of RLN on the larynx entrance during TTD surgery has the advantages of less injury and low complication rate,which can reduce the impact on the patient′s vocal cord function.
