4种镍钛根管预备器械清洁能力和牙本质微裂发生率比较 被引量:2

Cleaning ability and incidence of dentin microcracks with four nickel-titanium instruments for root canal preparation

摘要 目的:比较4种镍钛根管预备器械的根管清理能力和牙本质微裂发生率。方法:收集150颗具有2个独立根管的上颌前磨牙为器械组,分别用ProTaper Gold(PTG组)、ProTaper Next(PTN组)、自调节根管锉(SAF组)和ProTaper Universal(PTU组)进行根管预备,每组随机选取15个离体牙纵向劈开,扫描电镜评估根管内壁的碎屑和玷污层情况。将4组器械组剩余样本(n=20)和对照组(n=10)离根尖3、6、9 mm处横断牙根,置于体视显微镜下评估牙本质微裂情况。结果:4组器械组碎屑评分比较,差异有统计学意义(P<0.05);其中SAF预备根管后的碎屑评分最低,但组间两两比较,差异无统计学意义(P>0.05)。4组器械组玷污层评分比较,差异无统计学意义(P>0.05)。4组器械组牙本质微裂发生率比较,差异有统计学意义(P<0.05),其中PTU组牙本质微裂发生率较SAF组高(P<0.05)。结论:PTG、PTN、SAF和PTU预备上颌前磨牙根管的根管清理能力相当,但PTU预备根管后牙本质微裂发生率比SAF高。 Objective:To compare the cleaning ability and incidence of dentin microcracks with four nickeltitanium instruments for root canal preparation.Methods:A total of 150 maxillary premolars with 2 independent root canals were included as the instrument group.ProTaper Gold(PTG group),ProTaper Next(PTN group),self-adjusting file(SAF group)and ProTaper Universal(PTU group)were used for root canal preparation.Fifteen teeth ex vivo were randomly selected from each group,split longitudinally,and subjected to electronic microscope scanning for evaluation of debris and smear layer on the inner wall of root canal.The remaining samples from each instrument group(n=20 each)and the control group(n=10)were transected at 3,6,9 mm from the apex,and studied under a stereo microscope to evaluate the dentin microcracks.Results:There was a statistical difference in the debris scores among the four instrument groups(P<0.05).Specifically,the debris scores was the lowest with SAF preparation of root canals,but the between-group difference did not reach the level of statistical significance(P>0.05).There was no significant difference in the smear layer scores between the four groups(P>0.05).The four instrument groups differed statistically in incidence of dentin microcracks(P<0.05).The incidence of dentin microcracks in the PTU group was higher than that in the SAF group(P<0.05).Conclusion:PTG,PTN,SAF and PTU exhibited similar cleaning ability for root canal preparation of maxillary premolar.However,the incidence of dentin microcracks after root canal preparation is higher using PTU compared with SAF.
