摘要
目的评价标准化3D打印多孔钛合金髋臼杯全髋关节置换术的中期临床疗效。方法回顾性分析2016年2月至2018年8月于北京大学国际医院骨科行标准化3D打印多孔钛合金髋臼杯全髋关节置换术患者临床病历资料23例(25髋)。疗效评价指标包括术后并发症发生情况和假体存活率,以及术后6个月和末次随访VAS疼痛评分、Harris评分、髋臼侧影像评价及臼杯存活率。随访39~69(51.7±10.7)个月。结果23例(25髋)上述髋关节置换术后无并发症发生,假体存活率100%。VAS评分术后6个月为(1.44±0.82),末次随访时为(0.08±0.27),较术前(7.48±0.96)均显著降低(P<0.05)。Harris评分术后6个月为(86.88±3.52),末次随访时为(95.04±1.86),较术前(45.64±4.55)均显著提高(P<0.05)。影像学评估透亮带是否出现两组术后VAS评分及Harris评分差异无统计学意义。结论标准化3D打印多孔钛合金髋臼杯全髋关节置换术的中期临床疗效良好,具有髋臼存活率高及降低假体无菌性松动的优势。
Objective To evaluate the mid-term clinical efficacy of total hip replacement using a standardized 3 D printed porous trabecular titanium acetabular cup.Methods Clinical data of 23 consecutive patients undergoing total hip replacement with a standardized 3 D printed porous trabecular titanium acetabular cup(25 arthroplasties)between February 2016 and August 2018 were collected and retrospectively analyzed in this study.All patients were followed up for 39~69(51.7±10.7)months.Efficacy evaluation indicators included incidence of postoperative complications and survival rate of prosthesis,as well as visual analog scale(VAS)score,Harris score,acetabular side imaging evaluation and cup survival rate at 6 months after operation and last follow-up.Results No complications were observed in all 25 hip replacements,and the survival rate of cup was 100%during the whole follow-up.The VAS scores were decreased significantly at 6 months postoperatively(1.44±0.82)and last follow-up(0.08±0.27)when compared with the scores preoperatively(7.48±0.96,P<0.05).The Harris scores were increased significantly at 6 months postoperatively(86.88±3.52)and last follow-up(95.04±1.86)than the pre-operative scores(45.64±4.55,P<0.05).There were no significant differences in postoperative VAS score and Harris score between the patients with and without periprosthetic radiolucent zone.Conclusion Standardized 3 D printed porous trabecular titanium acetabular cup shows its favorable mid-term outcome in total hip arthroplasty,provides high acetabular survival rate,and has certain advantages in reducing aseptic loosening of the prosthesis.
