摘要
目的探讨不同分娩方式对初产妇盆底肌功能的影响,同时评价应用电刺激生物反馈疗法治疗不同分娩方式初产妇盆底功能的治疗效果。方法选取2020年12月-2022年8月就诊的产后42-56 d初产妇为研究对象,按分娩方式不同分为阴道分娩组及剖宫产组,观察不同分娩方式对盆底肌电生理功能的影响,同时观察阴道分娩治疗组及剖宫产治疗组治疗后盆底电生理功能的变化,并分析其对盆底功能的影响。结果剖宫产组产妇静息状态下肌肉的收缩变化(前基线)、快肌的功能(快速收缩最大值)、慢肌的肌力(5-10 s持续收缩平均值)、慢肌的耐力(60 s持续收缩平均值)及运动后肌肉的状态(后基线)均较阴道分娩组肌力增高,且差异有统计学意义(P<0.05);阴道分娩治疗组产妇治疗后阴道肌肌力及耐力均较治疗前有显著升高,且差异有统计学意义(P<0.05);剖宫产治疗组慢肌肌力较治疗前有显著升高,且差异有统计学意义(P<0.05)。结论阴道分娩相对于剖宫产更易造成盆底肌功能损伤,且电刺激生物反馈疗法联合盆底肌锻炼对产后早期盆底功能恢复具有显著疗效。
Objective To explore the effect of different delivery modes on the pelvic floor muscle function of primipara,and to evaluate the therapeutic effect of electrical stimulation biofeedback therapy on pelvic floor function of primipara with different delivery modes.Methods The 42-56 days postpartum primipara who were treated in Harbin 242 Hospital from December 2020 to August 2022 were selected as the study subjects,they were divided into vaginal delivery group and cesarean section group according to different delivery methods.The effects of different delivery methods on the electrophysiological function of pelvic floor muscles were observed.At the same time,the changes of pelvic floor electrophysiological function in the vaginal delivery treatment group and cesarean section treatment group after treatment were observed,and their effects on pelvic floor function were analyzed.Results The change of muscle contraction(pre baseline),the function of fast muscle(maximum value of fast contraction),the muscle strength of slow muscle(average value of 5-10s continuous contraction),the endurance of slow muscle(average value of 60s continuous contraction)and the muscle state after exercise(post baseline)of the cesarean section group were higher than those of the vaginal delivery group(P<0.05);The vaginal muscle strength and endurance of the parturients in the vaginal delivery treatment group after treatment were significantly higher than those before treatment(P<0.05);The slow muscle strength of cesarean section treatment group was significantly higher than that before treatment(P<0.05).Conclusions Compared with cesarean section,vaginal delivery is more likely to cause pelvic floor muscle function damage,and electrical stimulation biofeedback therapy combined with pelvic floor muscle exercise has a significant effect on the early recovery of pelvic floor function.
