摘要
目的了解新型冠状病毒肺炎(Coronavirus Disease 2019,COVID-19)患者临床特点,为临床工作提供指导。方法对中部战区总医院2020年1月10日至2020年2月20日收治的COVID-19确诊病例38例的一般情况、临床表现、实验室检查、影像学检查进行总结和统计学分析。结果38例患者中,发热、咳嗽、乏力为最主要的临床表现,纳差、恶心/呕吐、腹泻也占有一定比例。多有白细胞计数正常或减少、淋巴细胞计数减少;PCT未见明显升高,IL-6、CRP、铁蛋白、ESR等炎症指标均可升高;存在肝损伤、心肌损伤,未见明显肾损伤。以双肺感染居多,以磨玻璃影、斑片影主要表现。重症组的淋巴细胞计数更低,炎症指标(IL-6、CRP、铁蛋白)升高比例更高,出现肝损伤、心肌损伤的比例更高,差异均有统计学意义(P<0.05)。结论COVID-19患者重症组的淋巴细胞计数更低,IL-6、CRP、铁蛋白升高比例更高,更易出现肝损伤、心肌损伤。
Objective To understand novel coronavirus pneumonia(Coronavirus Disease 2019,COVID-19)clinical characteristics,and provide guidance for clinical work.Methods The general information,clinical manifestations,laboratory examination and imaging examination of 38 patients with covid-19 admitted to central theater general hospital from January 10,2020 to February 20,2020 were summarized and statistically analyzed.Results Among the 38 patients,fever,cough and fatigue were the main clinical manifestations.Anorexia,nausea/vomiting and diarrhea also accounted for a certain proportion.Most of them had normal or decreased white blood cell count and decreased lymphocyte count;PCT did not significantly increase,but IL-6,CRP,ferritin,ESR and other inflammatory indexes were increased;liver injury and myocardial injury were found,and no obvious renal injury was found.Most of the cases were double lung infection,and the main manifestations were ground glass shadow and patch shadow.In the severe group,the lymphocyte count was lower,the proportion of inflammatory indexes(IL-6,CRP,ferritin)was higher,the proportion of liver injury and myocardial injury was higher,the differences were statistically significant(P<0.05).Conclusion The severe group of covid-19 patients has lower lymphocyte count,higher proportion of IL-6,CRP and ferritin,and more prone to liver injury and myocardial injury.
