摘要
目的基于ELISA检测技术,探讨恙虫病患者血清细胞因子水平变化,为探索其临床意义及作用机制提供科学依据。方法收集确诊患者血清和健康人血清。利用ELISA技术检测外周血清8种细胞因子表达水平。采用SAS 9.1软件进行统计学分析。结果病例组中IL-8、IFN-γ、MCP-1、TNF-α等4种细胞因子浓度的中位数分别为34.09 pg/mL、17.58 pg/mL、181.42 pg/mL、33.49 pg/mL,表达水平较对照组有不同程度升高(P<0.05);病例组RANTES浓度的中位数为8.69 pg/mL,较对照组低(P<0.05);其余细胞因子表达水平在两组间差异无统计学意义(P>0.05)。重症组患者的血清IFN-γ、MCP-1、TNF-α浓度中位数分别为34.57 pg/mL、299.41 pg/mL、103.00 pg/mL,表达水平比轻症组患者明显升高(P<0.05),但血清IL-2、IL-8、IL-10、TNF-β、RANTES水平在重症组和轻症组患者之间差异无统计学意义(P>0.05)。IL-8与TNF-β(r=0.703,P=0.001)呈现正相关性,但其余细胞因子(IL-2、IL-10、IFN-γ、MCP-1、TNF-α、RANTES)之间无相关性。结论恙虫病东方体感染机体会导致多种血清细胞因子水平改变。血清IFN-γ、MCP-1,TNF-α水平的含量可以反映病情严重程度,对恙虫病病情评估具有重要的临床应用价值。
Objective To explore the changes of serum cytokine levels in patients with scrub typhus based on ELISA,and provide the scientific basis for exploring its clinical significance and mechanism of action.Methods The serum of patients and healthy people were collected.The expression levels of eight cytokines in peripheral serum were detected by ELISA.SAS 9.1 software was used for statistical analysis.Results The expression levels of four cytokines(IL-8,IFN-γ,MCP-1,TNF-α)in the patients’group(the median of the concentration of four cytokines was 34.09 pg/mL,17.58 pg/mL,181.42 pg/mL,33.49 pg/mL,respectively)were higher than those in the healthy group,while RANTES level(the median of RANTES expression was 8.69 pg/mL)was lower than that in the healthy group(P<0.05).There was no significant difference in the expression levels of other cytokines between the two groups(P>0.05).The serum levels of IFN-γ,MCP-1,and TNF-αin the severe group(the median of IFN-γ,MCP-1,and TNF-αwas 34.57 pg/mL,299.41 pg/mL,103.0 pg/mL,respectively)were significantly higher than those in the mild group(P<0.05),but the serum levels of IL-2,IL-8,IL-10,TNF-β,and RANTES were not significantly different between severe group and mild group(P>0.05).There was a positive correlation between IL-8 and TNF-β(r=0.703,P=0.001),but no correlation between other cytokines(IL-2,IL-10,IFN-γ,MCP-1,TNF-α,RANTES).Conclusion Orientia tsutsugamushi infection can lead to changes in serum cytokines.Serum levels of IFN-γ,MCP-1,and TNF-αcan reflect the severity of scrub typhus,which has important clinical application value in the assessment of scrub typhus.
