摘要
目的评价河南省乙型病毒性肝炎(乙肝)监测点病例报告质量,为改善乙肝监测质量提供参考。方法从中国疾病预防控制信息系统(NNDRS)导出2013-2019年河南省6个乙肝监测点报告的乙肝个案及附卡信息,对病例分类、附卡信息、抗-HBc IgM 1:1000检测结果进行分析和统计。结果6个监测点共报告乙肝病例22557例,剔除重复报告383例(1.70%),实际报告有效病例22174例,其中现住址为本县区的12743例,年均报告发病率在30/10万~60/10万之间,与监测前的2012年报告发病率(169.87/10万)相比,下降明显。报告病例中以确诊病例为主,占94.94%;急性乙肝1050例,采集血清标本817份,采样率77.81%,抗-HBc IgM 1:1000阳性673份,急性乙肝诊断准确率为64.10%。结论监测点乙肝报告发病率明显下降,报告病例更能接近真实发病水平,同时,进一步提高未能明确诊断为慢性乙肝病例的血清标本采集率,开展抗-HBc IgM 1:1000检测,可提高急性乙肝诊断的准确性。
Objective To assess the surveillance quality of hepatitis B surveillance pilots in Henan province,and provide a reference for improving the quality of hepatitis B surveillance system.Method According to the date of onset and current address,the information on hepatitis B cases and their attached cards of 6 Hepatitis B surveillance pilots in Henan province from 2013 to 2019 was collected from the National Notifiable Reporting System.The statistical analysis was done for the classification of cases,and the attached card information.Results A total of 22557 cases of hepatitis B were reported in the 6 surveillance pilots,22174 effective cases were reported after 383 cases(1.70%)of repeated reports were excluded.Among them,12743 cases were reported current address as the local county.The average annual incidence rate was between 30/100000 to 60/100000 from 2013 to 2019,compared with the reported incidence rate in 2012(169.87/100000),it has declined significantly.The reported cases were mainly confirmed cases,accounting for 94.94%.1050 cases were acute hepatitis B,of which 817 serum samples were collected,sampling rate was 77.81%,673 samples were anti-HBc IgM 1:1000 positive,and the diagnostic accuracy of acute hepatitis B was 64.10%.Conclusion The incidence of hepatitis B in surveillance pilots has decreased significantly,and the reported cases are approaching to the true level.At the same time,further improving the collection rate of serum samples in cases which failed to be diagnosed as chronic hepatitis B and carrying out anti-HBc IgM 1:1000 test can improve the accuracy of diagnosis of acute hepatitis B.
