老年晚期NSCLC患者放疗低剂量区与急性RP关系的研究 被引量:1

Relationship between low dose region and acute radiation pneumonia in elderly patients with advanced non-small cell lung cancer

摘要 目的:为了解老年晚期非小细胞肺癌(NSCLC)患者放疗低剂量区与急性放射性肺炎(RP)的关系,探讨此特定人群行调强适形放疗(IMRT)后正常肺组织剂量-体积参数对急性RP的预测作用。方法:回顾性分析2017年9月~2019年9月老年晚期NSCLC患者共52例,记录剂量-体积参数、RP发生率及严重程度。多因素分析剂量体积大小与中-重RP之间的关系,并对独立影响因素采用ROC曲线分析寻找合适的预测界值。结果:52例患者中,0级RP发生率约为23.1%(12/52),1级RP发生率约为40.4%(21/52),2级RP发生率约为15.4%(8/52),3级RP发生率约为21.2%(11/52),无4级和5级RP发生,经ROC曲线分析,V5预测≥2级RP的合适界值为54%。结论:V5对≥2级RP的发生具有独立影响作用,建议老年NSCLC晚期非手术人群行IMRT时应注意低剂量体积的大小。 Objective To investigate the relationship between low dose region and acute radiation pneumonia(RP)in elderly patients with advanced non-small cell lung cancer(NSCLC),and to explore the predictive effect of dose-volume parameters of normal lung tissue on acute RP after intensity-adjusted conformal radiotherapy(IMRT)in this specific population.Methods A total of 52 elderly patients with advanced NSCLC from September 2017 to September 2019 were retrospectively analyzed,and dose-volume parameters,incidence and severity of RP were recorded.Multivariate analysis of the relationship between dose volume size and medium-heavy RP,and ROC curve analysis for independent influencing factors to find the appropriate predictive bound.Results Among the 52 patients,the incidence of grade 0 RP was about 23.1%(12/52),40.4%(21/52),15.4%(8/52),21.2%(11/52),no grade 4 and 5 RP.Conclusion V5 has an independent effect on the occurrence of RP≥2,and it is suggested that the size of low dose volume should be paid attention to when IMRT is performed in elderly patients with advanced non-small cell lung cancer.
