福建省莆田市登革热本地暴发流行病学及病原学特征

Epidemiological and pathogenic characteristics of dengue fever local outbreak in Putian City of Fujian Province

摘要 目的了解福建省莆田市多次出现登革热(DF)本地病例暴发的原因,为制定防控策略提供依据。方法收集并分析莆田市2007、2008、2014、2015、2019年5个DF暴发年度病例资料,扩增并测定相应年度本地登革病毒(DENV)毒株E基因序列,构建系统进化树分析。结果莆田市2007—2019年5个暴发年度共报告348例,本地病例多在8~10月,国外输入性DF病例均来自东南亚国家;系统进化树显示2007、2008、2014、2015年莆田本地暴发毒株均为DENV2,Cosmopolitan基因型,但分属不同的进化分支存在明显差异,2019年莆田登革热暴发毒株有DENV1基因型Ⅰ型和DENV4基因型Ⅱ型,溯源结果显示暴发毒株序列与东南亚国家的毒株序列最为接近。结论莆田市DF暴发主要由东南亚国家输入引起,应大力加强入境检疫工作。 Objective To understand the causes of multiple local outbreaks of dengue fever(DF)in Putian City of Fujian Province,and to provide evidence for developing prevention and control strategies.Methods Case data of five outbreak years in Putian City,i.e.,2007,2008,2014,2015and 2019,were collected and analyzed.Full-length E gene fragments of local dengue virus(DENV)strains in the corresponding years were amplified and sequenced.An E gene phylogenetic tree was constructed for analysis.Results A total of 348cases of DF were reported in five DF outbreak years from 2007 to 2019in Putian City.The peak time of indigenous cases was from August to October.All the imported cases were from southeast Asian countries.Results of phylogenetic analysis indicated that the local strains in 2007,2008,2014and 2015 were DENV2,which were solely assigned to the same genotype Cosmopolitan,but belonging to different evolutionary branches.The local strains in 2019included DENV1(genotype I)and DENV4(genotype II).Tracing results showed that sequences of local outbreak strains were most similar to that of the southeast Asian countries.Conclusions Local outbreaks of DF in Putian City were mainly caused by imported cases from southeast Asian countries.Entry quarantine targeting these countries shall be strengthened.
