摘要
目的分析CT血管成像技术(computerized tomography angiography,CTA)联合磁共振成像(magnetic resonance imaging,MRI)技术在胰十二指肠切除治疗胰头癌中的临床应用价值。方法选取哈尔滨医科大学附属第四医院2018年12月~2021年12月收治的行胰十二指肠切除术治疗的胰头癌患者80例作为研究对象,随机分为对照组和研究组,每组40例。对照组在胰十二指肠切除术前采用常规CTA,研究组在胰十二指肠切除术前采取MRI联合CTA,对比临床疗效及不良反应。结果对照组平均手术时间(472.5±13.5)min、平均输血量(704.1±16.5)mL、平均住院时间(20.4±3.2)d、R0切除率77.5%(31/40)、并发症发生率20%(8/40)。研究组平均手术时间(345.8±20.5)min、平均输血量(604.1±30.1)mL、平均住院时间(12.3±3.3)d、R0切除率97.5%(39/40)、并发症发生率5%(2/40)。研究组和对照组比较,差异均具有统计学意义(均P<0.05)。结论CTA联合MRI技术应用于胰十二指肠切除术治疗胰头癌可显著缩短手术时间,降低输血量,缩短住院时间,提高R0切除率,减少并发症,改善患者预后。
Objective To analyze the clinical application value of computerized tomography angiography(CTA)combined with magnetic resonance imaging(MRI)in the treatment of pancreatic head cancer by pancreaticoduodenectomy.Methods From December 2018 to December 2021,80 patients with pancreatic head cancer treated by pancreaticoduodenectomy in the Fourth Affiliated Hospital of Harbin Medical University were selected as the study object.They were randomly divided into control group and experimental group(n=40).In the control group,pancreaticoduodenal resection was performed by conventional CTA,and in the experimental group,pancreaticoduodenal resection was performed by MRI combined with CTA.Results In the control group,the average operation time was(472.5±13.5)min,the average volume of blood transfusion was(704.1±16.5)mL,the average hospitalization time was(20.4±3.2)d,resection rate of R0 was(77.5%)(31/40),complication rate was 20%(8/40).In the experimental group,the average operation time was(345.8±20.5)min,the average blood transfusion was(604.1±30.1)mL,the average hospitalization time was(12.3±3.3)d,resection rate of R0 was 97.5%(39/40)and complication rate was 5%(2/40).There were significant difference between the experimental group and the control group(all P<0.05).Conclusion CTA combined with MRI in pancreaticoduodenectomy can significantly shorten the operation time,reduce the volume of blood transfusion,shorten the hospitalization time,improve the R0 resection rate,reduce complications,and improve the prognosis of patients.
