80 kV低电压联合iDose4迭代重建技术在儿童胸部CT检查中的应用研究

摘要 目的探讨基于80 kV低电压联合iDose4迭代重建技术进行儿童胸部CT扫描,即低剂量儿童胸部CT检查方案的临床可行性。方法选取哈尔滨医科大学附属第六医院影像中心2023年1月~2月98例行胸部CT检查的儿童,平均分为A、B两组,每组49例;A组采用常规电压(120 kV)进行扫描,B组采用低电压(80 kV)进行扫描,两组均采用自动管电流调节技术(automatic tube current modulation,ATCM)。其中A组行滤波反投影(filtered back projection,FBP)重建;B组采用不同的重建技术,依次行FBP、iDose4-1、iDose4~4、iDose4~6重建,记为B1、B2、B3、B4组。对A、B两组图像进行主客观质量比较。客观图像质量采用统计辐射剂量参数,包括容积CT剂量指数(volume CT dose index,CTDIvol)、剂量长度乘积(dose length product,DLP)、有效辐射剂量(effective dose,ED);选取气管分叉层面测量降主动脉CT值、噪声(standard deviation,SD),并计算信噪比(signal-to-noise ratio,SNR)来进行评价;主观图像质量评价按照五级评分法依次对A组、B1组、B2组、B3组、B4组图像的肺窗内各结构成像效果进行评分。结果客观图像质量CTDIvol、DLP、ED等参数B组相较于A组明显降低(P<0.05),剂量减低指数(dose reduction factor,DRF)为62.42%;降主动脉CT值各组间比较无明显差异(P>0.05);主观质量评价方面B1、B2、B3、B4、A组图像主观质量评分依次增高,B4组主观图像质量与A组相近,差异有统计学意义(P<0.05)。结论在儿童胸部CT扫描方案中,采用80 kV管电压联合iDose4~6重建技术可获得与120 kV管电压相当的主观图像质量,但在客观上可使儿童所接受的辐射剂量降低约62%,值得进一步临床推广。 Objective To investigate the clinical feasibility of low dose chest CT of the 80 kV low voltage combined with iDose4 iterative reconstruction technique for CT scanning of children’s chest.Methods Ninety-eight children who took chest CT examination in Imaging Center of the Sixth Affiliated Hospital of Harbin Medical University from January to February 2023 were selected and divided into two groups on average,with 49 cases in each group.The group A was performed with conventional voltage(120 kV)scanning and group B was performed with low-voltage(80 kV)scanning.Automatic tube current modulation(ATCM)was used in both groups.The conventional voltage group(group A)adopted filter back projection(FBP)reconstruction.The low-voltage group(group B)adopted the following different reconstruction techniques:FBP,iDose4~1,iDose4~4 and iDose4~6,which were labeled as group B1,B2,B3 and B4,respectively.Objective image quality was evaluated by statistical radiation dose parameters,including volume CT dose index(CTDIvol),dose length product(DLP),and effective radiation dose(ED);and tracheal bifurcation layer were chosen to measure the CT value of descending aorta,standard deviation(SD),and signal-to-noise ratio(SNR)was calculated for evaluation.The subjective quality of the images was evaluated according to the 5-level scoring method to score the imaging effect of each structure in the lung window of the five groups of images in group A,group B1,group B2,group B3,and group B4.Results After statistical analysis,CTDIvol,DLP and ED of objective image quality in group B were significantly lower than those in group A(P<0.05),and the dose reduction factor(DRF)was 62.42%.There were no significant differences in CT values of descending aorta among all groups(P>0.05).In terms of subjective quality evaluation,the subjective image quality scores of groups B1,B2,B3,B4 and A increased successively,and the subjective image quality of group B4 was similar to that of group A,with statistical significance(P<0.05).Conclusion In children’s chest CT
