摘要
社会资本参与发展医疗卫生事业,不仅能够缓解政府融资压力,还能够为医疗卫生行业带来管理、技术及绩效上的提升。PPP模式在医疗卫生领域的应用逐渐被重视,而合理分配参与主体间的项目收益,是保障社会资本参与有效程度及项目顺利推进的关键。本文介绍了医疗PPP项目的特点,界定了医疗PPP项目收益分配的概念,结合文献搜索和专家采访建立医疗PPP项目收益分配影响因素指标体系,具体包括项目投入、合作贡献、项目产出和风险分担4个方面,构建基于改进Shapley值的医疗PPP项目收益分配模型,并通过某县人民医院分院PPP项目验证其可行性,该模型下参与者贡献度越大、承担风险越多,收益就会越多,此方法使收益分配更加公平客观,能有效促进社会资本方积极参与医疗项目。由此得到启示:建立收益分配动态调整机制,提高争议解决效率;适时启动奖励机制,调动社会资本积极性;制定具体可操作的监管措施,保障项目顺利实施。
The participation of social capital in the development of healthcare not only alleviates government financing pressure,but also brings management,technology,and performance improvements to the healthcare industry.The application of PPP model in the field of healthcare is gradually being valued,and the reasonable allocation of project benefits among participating entities is the key to ensuring the effectiveness of social capital participation and the smooth progress of projects.This article introduces the characteristics of medical PPP projects,defines the concept of profit distribution in medical PPP projects,and establishes an indicator system for the influencing factors of profit distribution in medical PPP projects through literature search and expert interviews.It specifically includes four aspects:project investment,cooperative contribution,project output,and risk sharing.A profit distribution model for medical PPP projects based on improved Shapley values is constructed,and its feasibility is verified through a PPP project at a branch of a county People's Hospital,Under this model,the greater the contribution of participants and the more risks they bear,the greater the benefits will be.This method makes the distribution of benefits more fair and objective,and can effectively promote the active participation of social capital in medical projects.This provides inspiration:establishing a dynamic adjustment mechanism for income distribution to improve the efficiency of dispute resolution;Timely activate reward mechanisms to mobilize the enthusiasm of social capital;Develop specific and actionable regulatory measures to ensure the smooth implementation of the project.
