上腔静脉起源房性心律失常电生理特征分析 被引量:3

摘要 目的探讨上腔静脉(SVC)起源房性心律失常的体表心电图及心内电生理特征。方法选取自2003年1月至2018年12月于北部战区总医院经心内电生理标测证实的61例起源于SVC的房性心律失常患者为研究对象,分析患者的体表心电图及心内电生理特征。结果房性早搏、房性心动过速时的P′波振幅在Ⅱ、Ⅲ、aVF导联时均显著高于窦律时P波,差异有统计学意义(P<0.05)。房性早搏时SVC电位提前冠状窦口电位时间长于窦律时,差异有统计学意义(P<0.05)。结论SVC起源房性心律失常有其特征性的体表心电图及心内电生理特点。在消融心房颤动或房性心动过速时,应注意识别SVC起源,以提高手术成功率。 Objective To investigate the surface electrocardiogram and electrophysiologic characteristics of atrial arrhythmias originating from superior vena cava(SVC).Methods A retrospective study was performed on 61 cases of patients with who underwent electrophysiologic examination of atrial arrhythmias originating from SVC from January 2003 to December 2018.The surface electrocardiogram(ECG)and electrophysiologic characteristics of the patients were analyzed.Results Compared with sinus P wave,the amplitude ofⅡ,Ⅲ,aVF leads in atrial premature P’wave was significantly higher,and the difference was statistically significant(P<0.05).In atrial premature beats,the time of SVC potential ahead coronary sinus ostium was longer than that in the sinus rhythm(P<0.05).Conclusion SVC originated atrial arrhythmia has its characteristic surface electrocardiogram and electrophysiologic characteristics.In the ablation of atrial fibrillation or atrial tachycardia,attention should be paid to identify the origin of SVC,which can improve the success rate of surgery.
