Differential Diagnosis of Benign and Malignant Renal Tumors by Multi-Phase CT Improved Enhancement Parameters

摘要 目的探讨基于CT平扫及三期增强的改良参数对肾脏良恶性肿瘤的诊断价值。方法回顾性分析197例肾恶性肿瘤及29例良性肿瘤患者术前初诊CT扫描图像,采用单因素分析比较肾恶性肿瘤与肾良性肿瘤组、肾恶性肿瘤与嗜酸细胞瘤组及肾恶性肿瘤与乏脂性血管平滑肌脂肪瘤组改良增强参数的差异并计算曲线下面积(AUC),探讨参数在肾良恶性肿瘤影像学鉴别诊断的价值。结果相较于传统CT值测量法,改良增强参数将AUC值提升0.1~0.16,其在鉴别肾恶性肿瘤与良性肿瘤、嗜酸细胞瘤及乏脂性血管平滑肌脂肪瘤最高AUC分别为0.805、0.844及0.826。其中排泄期相对主动脉的校正CT值诊断效能最高。结论改良增强参数对肾脏良恶性肿瘤鉴别的诊断效能优于传统CT值测量法。 Objective To explore the diagnostic value of improved parameters based on CT plain scan and three-phase enhancement for renal benign and malignant tumors.Methods Retrospective analysis of CT scan images of 197 renal malignant tumors and 29 benign tumor patients before operation was performed.Univariate analysis was used to compare renal malignant tumor with renal benign tumor group,renal malignant tumor with eosinophilic tumor group,and renal The difference between the improved enhancement parameters of malignant tumor and lipid-free vascular smooth muscle lipoma group was calculated and the area under the curve(AUC)was calculated to explore the value of parameters in the differential diagnosis of renal benign and malignant tumors.Results Compared with the traditional CT measurement method,the improved enhancement parameters increased the AUC value by 0.1~0.16.The highest AUC for distinguishing renal malignant tumors from benign tumors,eosinophilic tumors,and lipid-poor angiomyolipoma were 0.805,0.844,and 0.826.The corrected CT value of the excretory phase relative to the aorta has the highest diagnostic efficiency.Conclusion The improved diagnostic parameters are superior to traditional CT in the diagnosis of renal benign and malignant tumors.
