复发缓解型多发性硬化伴感觉障碍患者脑网络的研究 被引量:3

Study of Brain Network in Patients with Relapsing-Remitting Multiple Sclerosis with Sensory Disorders

摘要 目的采用功能网络方法分析复发缓解型多发性硬化(RRMS)伴感觉障碍患者的脑网络属性的异常改变。方法对26例RRMS伴感觉障碍患者和20名性别、年龄匹配的健康志愿者(对照组)均行静息状态功能磁共振成像(rs-fMRI),同时对扩展残疾状态量表(EDSS)评分、病程和Fugl-Meyer量表感觉功能评分(FmSS)评估,并探讨差异脑区的网络属性与临床评分的相关性。结果与对照组相比,RRMS组局部效率(Eloc)明显下降,主要包括双侧额上回、额中回、扣带回、楔叶、舌回、中央后回及右侧颞枕叶;RRMS度(Deg)下降的脑区主要包括双侧额上回、扣带回以及左侧额中回、左侧岛叶,差异均有统计学意义(P<0.01,FDR校正);RRMS组与对照组的全脑效率(Eg)无明显差异。Eloc与病程呈负相关,包括右侧眶部额上回及左侧额上回内侧(P<0.05)。结论脑网络的拓扑属性与临床之间的相关性表明,基于图论的复杂网络分析可以作为评估MS疾病活动的有效工具。 Objective We aimed to explore abnormal changes in brain network properties in Patients with relapsing-remitting multiple sclerosis with sensory impairment using functional network methods.Methods Resting-state functional MRI(RS-fMRI)was performed on 26 Patients with RRMS with sensory disturbance in our hospital and 20 gender-and age-matched healthy controls.The extended disability status score(EDSS),disease duration and Fugl-Meyer sensory function score(FmSS)assessment were also Performed and explore the correlation between network property and clinical scores.Results Compared with the HC group,the local efficiency of the RRMS group decreased significantly,including bilateral superior and medial frontal gyrus,cingulate gyrus,cuneus gyrus,lingual gyrus,postcentral gyrus and temporal and occipital lobe;the Deg of bilateral superior frontal gyrus,cingulate gyrus,left medial frontal gyrus and left insular lobes were decreased(P<0.01,FDR corrected);global efficiency between RRMS group and HC group were not obviously significant.Eloc was negatively correlated with the disease duration,including the right orbital,superior frontal lobe and the left medial superior frontal gyrus(P<0.05).Conclusion The correlation between the topological properties of the brain network and clinic indicates that the complex network analysis based on graph theory can be used as an effective tool for evaluating MS disease activity.
