摘要
目的探讨MRI在儿童系统性红斑狼疮(SLE)并发骨梗死诊断中的价值。方法回顾性分析2016年1月至2019年1月青岛市妇女儿童医院10例骨梗死患儿的临床和MRI影像资料,该组患儿均经本院或外院确诊为SLE。结果10例患儿的骨梗死区都位于下肢,共累及50个部位,28个长骨骨干-干骺端,16个骨骺,3个髌骨,2个距骨,1个跟骨;14个关节合并积液。骨梗死的MRI影像特征性改变为"双轨征"、"三环征"和"地图样"改变。结论儿童SLE并发骨梗死MRI具有一定特征性,综合不同序列MR图像的信号特点,可尽早评估梗死范围、程度,亦可与原发病的关节病变相鉴别,为临床制定治疗方案提供信息。
Objective To investigate diagnostic value of MRI for bone infarction in children with systemic lupus erythematosus.Methods Clinical and MRI data of 10 children with bone infarction in Qingdao women and children’s hospital from January 2016 to January 2019 were retrospectively analyzed.All children in this group were diagnosed as SLE by our hospital or other hospital.Results 10 chilren with bone infarction areas were located in the lower limbs.The location of bone infarction involved 50 sites,28 sites located on long tubular bone diaphysis-metaphysis,16 located on epiphysis,3 located on patella,2 located on talus and 1 located on calcaneus,14 joints with effusion.Typical MRI finding of bone infarcton:"double-line sign"、"three ring sign"and"map-like"change.Conclusion The MRI of bone infarction in children with systemic lupus erythematosus has certain characteristics.Integrated signal characteristics of different sequences of MRI were useful for evaluating the range and degree of bone infarction.MRI features are also helpful for determining whether there was a complication and providing information for clinical treatment.
