摘要
目的探讨多层螺旋CT血管三维重组造影评估髂内动脉分支的分型情况。方法回顾性分析2018年7月至2019年12月进行CT髂血管三维重组造影检查的180例患者资料。应用Yamaki分类方法进行初步分类,并在此分类基础上进行髂内动脉及其分支分型情况评估,分为A~D四组。结果在本研究所获得的所有图像中,髂内动脉Yamaki分类方法A组所占比例最高(70.5%),其次为B组(25.6%)和C组(3.9%),研究中未见到D组。在Yamaki分类A组及B组中,闭孔动脉、直肠中动脉及直肠下动脉起源均具有一定的差异(闭孔动脉χ^(2)=6.52,P=0.01;直肠中动脉χ^(2)=13.27,P<0.05;直肠下动脉χ^(2)=10.36,P=0.001)。结论术前应用多层螺旋CT血管造影能够识别髂内血管及其分支动脉起源、走行、变异等信息。
Objective To evaluate the variation of internal iliac artery branch with multislice computed tomography angiography(MSCTA).Methods A total of 180 patients who underwent MSCTA were retrospectively enrolled in this study(95 men and 85 women;mean age,57.0 years).Yamaki’s classification(four groups of A-D according to the main branches of iliac vessels)was used for preliminary classification,and variation of small vessel branches was evaluated based on this classification.Results Based on the Yamaki’s classification,group A was the most common one(70.5%),followed by group B(25.6%)and group C(3.9%).Group D was not found in this study.In Yamaki classification group A and group B,there were significant differences in the origin of the obturator artery,middle rectal artery,or inferior rectal artery(obturator arteryχ^(2)=6.52,P=0.01;Middle rectal arteryχ^(2)=13.27,P<0.05;Inferior rectal arteryχ^(2)=10.36,P=0.001.Conclusion Preoperative application of MSCTA three-dimensional reconstruction imaging can provide detailed information of origin and variation of the internal iliac vessels and their branches.
