摘要
目的探讨清胰化积方联合化疗对湿热毒蕴型晚期胰腺癌患者转化生长因子-β_(1)(TGF-β_(1))、基质金属蛋白酶-9(MMP-9)、CC型趋化因子18(CCL18)及免疫功能的影响。方法将2017年1月至2019年9月就诊的101例湿热毒蕴型晚期胰腺癌患者随机分为对照组50例和观察组51例,对照组采用吉西他滨+顺铂进行化疗,观察组在对照组治疗方案的基础上联合清胰化积方治疗。结果治疗后,观察组血清糖类抗原125(CA125)、癌胚抗原(CEA)、糖类抗原19-9(CA19-9)水平和血清MMP-9、CCL18、TGF-β_(1)水平低于对照组(P<0.05);观察组外周血CD4^(+)/CD8^(+)、CD4^(+)细胞、CD3^(+)细胞指标水平均高于对照组(P<0.05);观察组主要和次要中医症状积分均低于对照组(P<0.05);观察组获益率高于对照组(P<0.05);观察组不良反应总发生率低于对照组(P<0.05)。结论对于湿热毒蕴型晚期胰腺癌患者,在吉西他滨+顺铂化疗方案的基础上联合清胰化积方治疗有助于延缓疾病进展,提高临床疗效,增强患者免疫力,降低化疗不良反应的发生率。
AIM To investigate the effects of Qingyihuaji formula combined with chemotherapy on the levels of transforming growth factor β_(1)(TGF-β_(1)),matrix metalloproteinase 9(MMP-9),CC chemokine ligand 18(CCL18),and immune function in patients with advanced pancreatic cancer of the damp-heat toxin accumulation type.METHODS A total of 101 patients with advanced pancreatic cancer of the damp-heat toxin accumulation type,who were treated from January 2017 to September 2019,were randomly divided into a control group of 50 patients and an observation group of 51 patients.The control group received chemotherapy with gemcitabine and cisplatin,and the observation group received Qingyihuaji formula in addition to the treatment received by the control group.RESULTS After treatment,the levels of serum glycoantigen 125(CA125),carcinoembryonic antigen(CEA),glycoantigen 19-9(CA19-9),MMP-9,CCL18,and TGF-β_(1) in the observation group were lower than those in the control group(P<0.05),while the peripheral blood levels of CD4^(+)/CD8^(+),CD4^(+)cells,and CD3^(+)cells in the observation group were higher than those in the control group(P<0.05).The scores for both primary and secondary traditional Chinese medicine(TCM)symptoms in the observation group were lower compared to those in the control group(P<0.05).The benefit rate in the observation group was higher than that in the control group(P<0.05),and the overall incidence of adverse reactions was lower in the observation group than that in the control group(P<0.05).CONCLUSION For patients with advanced pancreatic cancer of the damp-heat toxin accumulation type,the combination of Qingyihuaji formula with gemcitabine and cisplatin chemotherapy helps to delay disease progression,improve clinical efficacy,enhance immune function,and reduce the incidence of chemotherapy-related adverse reactions.
