摘要
《三因极一病证方论》所载温胆汤的药物组成包括半夏、竹茹、枳实、陈皮、炙甘草、茯苓、生姜、大枣,其具有理气化痰、清胆和胃的功效,是治疗胆胃不和、痰热内扰证之常用方,在包括心血管科、呼吸科、消化科、肾病科在内等临床各科的治疗中得以广泛运用。刘小虹教授是广州中医药大学第一附属医院呼吸内科博士研究生导师、广东省名中医,从医30余载,在治疗肺系疾病时,能准确辨证,变通用药,对于以痰热作祟为核心病机的肺系疾病,常灵活运用温胆汤为底加减,每每取得良好的临床疗效,对此颇有治疗心得。该文通过整理门诊上具有代表性的刘小虹教授运用温胆汤加减治疗肺系疾病的验案验方,包括慢性咳嗽、支气管扩张症、慢性阻塞性肺疾病、肺结节病、胸腔积液案共5则,分析刘小虹教授运用温胆汤治疗肺系疾病的临床经验,以飼同道,以期为肺系疾病的中医药治疗提供借鉴和参考。
Wendan Decoction(温胆汤)in Sanyi Jiyi Bingzheng Fang includes Banxia(Pinelliae Rhizoma),Zhuru(Bambusae Caulis In Taenias),Zhishi(Aurantii Fructus Immaturus),Chenpi(Citri Reticulatae Pericarpium),Zhigancao(Glycyrrhizae Radix Et Rhizoma Praeparata Cum Melle),Fuling(Poria),Shengjiang(Zingiberis Rhizoma Recens),Dazao(Jujubae Fructus),which has the effects of regulating Qi and resolving phlegm,clearing gallbladder and stomach.It is a common prescription for the treatment of gallbladder-stomach disharmony and phlegm-heat internal disturbance syndrome,and is widely used in clinical treatment,including cardiovascular department,respiratory department,digestive department,nephrology and other clinical departments.Professor LIU Xiaohong is a doctoral supervisor in the Department of Respiratory Medicine in the First Affiliated Hospital of Guangzhou University of Chinese Medicine and a famous traditional Chinese medicine in Guangdong Province.In the treatment of lung diseases,she can accurately differentiate symptoms and signs and flexible use of drugs.For lung diseases with phlegm-heat as the core pathogenesis,she often flexibly uses Wendan Decoction and often obtains a good clinical effect,and has a lot of experience in the treatment of this disease.By sorting out Professor LIU Xiaohong′s representative prescriptions for the treatment of pulmonary diseases with Wendan Decoction in the outpatient clinic,including 5 cases of chronic cough,bronchiectasis,chronic obstructive pulmonary disease,pulmonary sarcoidosis and pleural effusion.This paper analyzed professor LIU Xiaohong′s clinical experience in the treatment of pulmonary diseases with Wendan Decoction,in order to provide reference for the treatment of pulmonary diseases with traditional Chinese medicine.
