摘要
问诊是中医四诊的重要组成部分,在临床辨证治疗中具有重要意义。儿科又称"哑科",对医者而言儿科问诊尤难把握。在临床实践中清晰的问诊思路是诊治疾病的关键之一。在中医分科发展的过程中有关儿科问诊的专科特点渐显不足,加之小儿在生理病理上与成人不同,问诊方式与内容上亦有所不同。儿科问诊主要采用直接问诊与间接问诊相结合的方法。在小儿肺系疾病问诊时以"热、咳、食、便"四字为领,再分以问诊要点与伴随症状两部分,兼问体质、旧疾与服药等,直击要点,层次分明,有助于全面把握病情。既能突出肺系疾病的临床特征,又结合了小儿生理病理的特殊性。以期能更好地体现儿科问诊的专科性,为中医儿科临床诊治疾病提供更丰富的思路与方法。
Inquiry is an important part of the four traditional Chinese medicine(TCM)diagnosis methods,which is of great significance in clinical syndrome differentiation and treatment.Pediatrics is also called“dumb department”.For doctors,pediatric inquiry is particularly difficult to grasp.Clear inquiry thinking is one of the keys to diagnose and treat disease in clinical practice.In the course of the development of traditional Chinese medicine branch,the specialized subject characteristic of pediatric inquiry gradually shows deficiency.In addition,children are different from adults in physiology and pathology,and so are the ways and contents of inquiry.Pediatric inquiry mainly combines direct inquiry with indirect inquiry.In the inquiry of pediatric lung diseases,the main theme is“heat,cough,food and stool”.Each part is divided into inquiry points and accompanying symptoms,combined with constitution,old illness and medication and so on,directly to the point.The level is clear and it is helpful to grasp the situation comprehensively.It can not only highlight the clinical characteristics of lung diseases,but also combine the particularity of physiology and pathology in children.It is hoped that it can better reflect the specialized subject characteristic of pediatric inquiry and provide more thoughts and methods for the clinical diagnosis and treatment of diseases in TCM pediatrics department.
