摘要
梳理明清以前关于痞满的文献,梳理出痞满病因、病机理论框架,发掘痞满病因在空间维度上分为外感病因和内伤病因,外感病因最主要病因为寒邪,其他外感因素如:外感暑邪、湿邪太盛、痰饮、食积、痧秽等多种病因均可以导致痞满发生。内伤病因包括饮食所伤、七情失和、病理产物病因、及其他病因导致的痞满。文章梳理出饮食所伤的病因主要是指饮食偏嗜甜美和煎烤之物;七情中“怒”最易致痞;病理产物病因主要是痰饮所致痞满;其他病因主要是指误下和克伐太过。痞满的病机主要分为脏腑病机、六经病机、三焦病机、气血津液病机和阴阳失调病机。通过系统梳理明清以前医家关于痞满病因病机的著作,构建痞满病因病机基本理论框架,为临床诊治痞满提供理论依据。
Combing the literature on distention and fullness before the Ming and Qing Dynasties,combing out the theoretical framework of the etiology and pathogenesis of distention and fullness and discovering that the etiology of distention and fullness is divided into exogenous and internal causes.The main exogenous causes include cold evil.Other exogenous factors such as exogenous heat evil,excessive dampness evil,phlegm drinking,food accumulation,Sha evil and so on can lead to distention and fullness.The internal causes include diet injury,seven emotions disharmony,pathological products and fullness caused by other etiologies.The article combs out that the cause of diet injury mainly refers to the preference for sweet and fried food.Among the seven emotions,“anger”is the most likely to cause distention.The main etiology of pathological product is fullness caused by phlegm.Other causes mainly refer to the purging wrong and too much attacking drugs.The pathogenesis of distention and fullness is mainly divided into Zang-Fu organs pathogenesis,six meridians pathogenesis,triple energizer pathogenesis,Qi,blood and body fluid pathogenesis and Yin-Yang imbalance pathogenesis.By systematically combing the works of doctors on the etiology and pathogenesis of distention and fullness before the Ming and Qing Dynasties,this paper constructs the basic theoretical framework of the etiology and pathogenesis of distention and fullness,so as to provide a theoretical basis for the clinical diagnosis and treatment of distention and fullness.
