摘要
目的观察应用水针刀松解的方法治疗腰背肌筋膜炎的临床疗效及对腰背部疼痛和功能障碍的影响。方法将所选取的腰背肌筋膜炎患者40例随机分成两组:治疗组(水针刀治疗组)20例,应用水针刀松解方法治疗,每7 d治疗1次,治疗3次为1个疗程,共15 d。对照组(物理治疗组)20例,采用腰背部手法按摩、负压低频、微波的治疗方法,此三种疗法每日各1次,连续治疗15次为1个疗程,共15 d。分别观察治疗前、第一次治疗后24 h、第9天、第16天的腰背部疼痛VAS评分及Oswestry功能障碍指数(0swestry Disability Index,ODI)评分。以及在治疗前、后birddog体位多裂肌横截面积的变化。结果治疗前两组患者腰背部疼痛VAS评分及ODI评分差异无统计学意义(P>0.05)。第一次治疗后24 h、第9天、第16天,水针刀治疗组VAS评分及ODI评分均明显低于物理治疗组(P<0.05)。治疗结束后(第16天)水针刀治疗组多裂肌横截面积显著缩小(P<0.05)。治疗结束后(第16天)水针刀治疗组总有效率为95%,明显高于物理治疗组(P<0.05)。结论水针刀治疗腰背肌筋膜炎疗效显著,明显缓解腰背部疼痛及功能障碍,值得推广和应用。
Objective To explore the clinical effect of water needle knife in the treatment of chronic low back pain.Methods 40 cases of low back myofascitis patients were randomly divided into two groups:the treatment group(water needle knife treatment group)20 cases,the application of water needle knife release method treatment,once every 7 days,treatment for 3 times as a course of treatment,a total of 15 days.The control group(physical therapy group)of 20 cases,using low back massage,negative pressure low frequency,microwave treatment,the three kinds of treatment,once a day,15 times for a course of treatment,a total of 15 days.VAS score and Oswestry disability index(ODI)score of low back pain were observed before treatment and 24 hours,9 days and 16 days after the first treatment.The cross-sectional area of multifidus muscle in birddog position was measured before and after treatment.Results There was no significant difference in VAS score and ODI score between the two groups before treatment(P>0.05).24 hours,9 days and 16 days after the first treatment,the VAS score and ODI score of the water needle knife treatment group were significantly lower than those of the physical treatment group(P<0.05).At the end of treatment(the 16 th day),the cross-sectional area of multifidus muscle in the water needle knife treatment group was significantly reduced(P<0.05).After treatment(the 16 th day),the total effective rate of the water needle knife treatment group was 95%,which was significantly higher than that of the physical treatment group(P<0.05).Conclusion Water needle knife has a significant effect in the treatment of low back myofascitis,which can significantly relieve low back pain and dysfunction,and is worthy of promotion and application.
