针刺触发点对肌筋膜疼痛综合征大鼠痛阈改变的机制研究 被引量:3

Mechanism of acupuncture trigger point treatment on pain threshold changes in rats with myofascial pain syndrome

摘要 目的观察针刺对慢性肌筋膜疼痛(MPS)模型大鼠痛阈值的影响,分析针刺治疗MPS的潜在病理生理机制是否与细胞外信号调节激酶(ERK)蛋白有关。方法随机将24只雄性SD大鼠分为对照组、MPS组、阳陵泉(GB34)组和肌筋膜触发点(MTrPs)组,每组6只。对照组正常喂养,MPS组、GB34组和MTrPs组采用钝性打击SD大鼠左侧腓肠肌结合离心运动的方式建立MPS模型,期间对大鼠进行8周造模干预和4周的恢复期。造模完成后,分别对GB34组和MTrPs组大鼠左下肢阳陵泉穴和触发点进行电针干预,电流强度以大鼠左下肢轻微震颤为宜,留针15min,1次/d,连续7d。结果MPS组大鼠腓肠肌形态发生改变,病理横切面可见大小不等的圆形肌纤维,纵切面可见粗细相间的梭形肌纤维。针刺治疗可提高大鼠机械缩足阈值和热缩足潜伏期,并降低MTrPs周围损伤肌纤维中ERK、p-ERK和肌球蛋白轻链激酶(MLCK)的表达。此外,治疗后MTrPs周围炎性细胞减少,血清肿瘤坏死因子-α(TNF-α)和白细胞介素-1β(IL-1β)水平降低。结论针刺治疗可改善大鼠机械缩足阈值和热缩足潜伏期,针刺触发点效果优于传统穴位,可能与改变局部ERK蛋白激酶表达有关。 Objective To observe the effect of acupuncture on rats with myofascial pain syndrome(MPS)and to analyze the correlation between potential pathophysiological mechanism of acupuncture treatment and the extracellular signal-regulated kinase(ERK)protein.Methods Twenty-four male SD rats were randomly divided into Control group,MPS group,Yanglingquan point(GB34)group and myofascial trigger points(MTrPs)group,with six rats in each group.The MPS model was established by blunt striking of the left gastrocnemius muscle combined with eccentric exercise for eight weeks followed by a four weeks recovery period.After the rat modeling process was completed,the GB34 group were treated with electroacupuncture(EA)at the left lower limb Yanglingquan point,while the MTrPs group were treated with electroacupuncture(EA)at the left lower limb trigger point.The appropriate current intensity was the slight tremor in the rats'left lower limb,and the needle was kept for 15 minute,once a day,for 7 days.Results The morphology of the gastrocnemius muscle(GM)were changed,and different sizes of the round muscle fibers in the transverse section and fusiform shapes in the longitudinal section were observed in the rats with MPS.The mechanical paw withdrawal threshold and thermal paw withdrawal latency in rats had increased,while the expression of ERK,p-ERK and myosin light chain kinase(MLCK)in the injured muscle fibers around MTrPs had decreased.In addition,inflammatory cells around MTrPs had decreased,and serum tumor necrosis factor-α(TNF-α)and interleukin-1β(IL-1β)levels had decreased.Conclusion Acupuncture treatment can improve the mechanical withdrawal thresholds and thermal withdrawal latency in rats.The effect of acupuncture at trigger points is better than that at traditional acupoints,which may be related to the change in the expression of local ERK protein kinase.
