摘要
目的:探讨基层医院利用血清前列腺特异性抗原(PSA)联合双参数磁共振成像(bpMRI)对前列腺癌的诊断价值。方法:收集黄陂区2018年1月至2019年12月因PSA升高行病理活检的307例男性患者资料,回顾性分析患者血清PSA和bpMRI图像,以病理活检结果为金标准,并通过ROC曲线预测血清PSA与bpMRI图像联合诊断前列腺癌的价值。结果:307例患者中,前列腺癌检出132例,检出率42.99%;血清PSA越高,前列腺癌检出阳性率高;bpMRI信号异常越明显,前列腺癌检出阳性率高。两者联合检测时曲线下的面积(AUC)为0.884,灵敏度0.775,约登指数0.702,均高于单独检测,特异度0.927与bmMRI图像单独检测相同。结论:PSA联合bpMRI在基层医院有限条件下,对临床诊断前列腺癌具有较高价值。
Objective:To evaluate the diagnostic value of primary healthcare institutions utilizing serum prostate specific antigen(PSA)combined with biparametric magnetic resonance imaging(bpMRI).Methods:Dataof 307 male patients with elevated PSA undergoing pathologic biopsy were collected from January 2018 to December 2019 in the area of Huangpi.Serum PSA and bpMRI images were retrospectively analyzed,the resultsof pathologic biopsy was the gold standard,and the diagnostic value of serum PSA combinedwith bpMRI images by ROC curves was predictedby received operating characteristic curve(ROC).Results:Among 307 patients screened by serum PSA,132 patients were diagnosed prostate cancer by prostate biopsy,with a detection rate of 42.99%;the higher the serum PSA rise,the higher the positive prostate cancer detection rate;the more pronounced the bpMRI abnormal signal,the higher the positive prostate cancer detection rate.The area under the curve(AUC)was 0.884 with a sensitivity of 0.775 and the iodine index was 0.702 for both assays,both of which were higher than the individual assays,and the specificity of 0.927 was the same for the individual bmMRI imagesassays.Conclusion:PSA combined with bpMRI is of high value in the clinical diagnosis of prostate cancer under limited conditions in primary hospitals.
