摘要
【目的】探讨因医疗行为不当所致医疗纠纷的成因及对策,为持续改进医疗质量、构建和谐融洽医患关系提供思路。【方法】以某三级综合医院2003年至2015年的221例医疗纠纷为研究对象,使用SPSS软件从时间序列分布变化和成分结构比较方面统计分析,进行统计描述;基于事故致因理论建立医疗行为安全模型,采用Pareto分析患者死亡后出现纠纷的医疗技术因素。【结果】医疗技术问题是导致医疗纠纷的重要原因,其中手术后并发症、神经血管介入检查治疗并发症、漏诊误诊居前三位。【结论】三级综合医院应以持续提高医疗技术能力水平作为防范医疗纠纷的主要策略,注重加强围手术期医疗安全管理,对于疑难复杂和急危重症患者,应坚持多学科联合诊疗,以有效防范差错事故。
【Objective】To explore the causes and countermeasures of medical disputes caused by improper medical behaviors,and provide ideas for continuous improvement of medical quality and establishment of a harmonious relationship between doctors and patients.【Methods】A total of 221 medical disputes in a third-level general hospital from 2003 to 2015 were selected as the research objects.SPSS software was used for statistical analysis according to the time series distribution and component structure.Based on the dispute cause theory,behavioral safety model was established.Pareto was used to analyze medical technical factors after the patient’s death.【Results】Medical technical problems were the important causes of medical disputes,among which post-operative complications,neurovascular interventional treatment complications,and missed diagnosis and misdiagnosis ranked the top three.【Conclusion】Threelevel general hospitals should continue to improve the level of medical technology as the main strategy to prevent medical disputes,and focus on strengthening perioperative medical safety management.For the patients with complicated and critical diseases,multiple disciplinary team can prevent errors and accidents effectively.
