摘要
【目的】利用磁共振扩散张量成像(diffusion tensor imaging,DTI)对正常腓肠肌及损伤后的腓肠肌进行定量研究,评价腓肠肌内、外侧头的表观扩散系数(apparent diffusion coefficient,ADC)及部分各向异性(fractional anisotropy,FA)等参数变化的意义。【方法】选取2018年12月至2019年4月因小腿外伤到急诊检查的19例患者为研究组,10名健康志愿者为对照组,使用GE1.5T超导型全身磁共振扫描仪和8通道头颈相控阵联合线圈,对受试者行常规扫描序列及横断位DTI序列检查。在DTI序列上对腓肠肌内、外侧头分别选取感兴趣区(region of interest,ROI),分别测量各ROI的ADC值和FA值。评价正常腓肠肌和损伤腓肠肌的ADC和FA值差异。将损伤组按照损伤程度分为Ⅰ度损伤组和Ⅱ度损伤组,此两组与对照组之间应用单因素方差分析分别比较ADC值和FA值。【结果】与对照组相比,研究组腓肠肌内、外侧头ADC值明显升高,FA值明显降低,差异均有统计学意义(P<0.05)。对照组与Ⅰ度损伤组和Ⅱ度损伤组腓肠肌内、外侧头ADC值及FA值比较,差异均有统计学意义(P<0.05),Ⅰ度损伤组与Ⅱ度损伤组腓肠肌内、外侧头ADC值比较,差异有统计学意义(P<0.05),FA值比较差异无统计学意义(P>0.05)。【结论】DTI可用于对损伤后的腓肠肌定量分析,其参数与腓肠肌损伤程度有相关性,可为评价腓肠肌损伤提供定量依据。
【Objective】To quantitatively evaluate normal and injured gastrocnemius muscles by diffusion tensor imaging(DTI), and evaluate the changes of apparent diffusion coefficient(ADC) and fractional anisotropy(FA) of the medial and lateral heads of gastrocnemius muscles.【Methods】From December 2018 to April 2019, 19 patients with leg trauma undergoing leg MRI in the emergency department were selected as the patient group and 10 healthy volunteers as the control group. GE 1.5 T superconducting whole body magnetic resonance scanner and 8-channel head and neck phased array combined coil were used to perform the routine scanning sequence and cross-sectional DTI sequence. The region of interest(ROI) was placed in the medial and lateral head of gastrocnemius muscles on DTI sequence, and the ADC and FA values of each ROI were measured. The differences of ADC and FA between normal and injured gastrocnemius muscles were analyzed. According to the degree of injury, the patients in the patient group were divided into the first and second degree injury groups. The ADC and FA values were compared between the two groups and the control group by ANOVA.【Results】Compared with the control group, the ADC values of the medial and lateral heads of gastrocnemius muscles in the patient group were significantly higher and the FA values were significantly lower than those in the control group(P <0.05). There was significant difference in the ADC and FA values of the medial and lateral gastrocnemius muscles among the control group, first degree injury group and second degree injury group(P<0.05). There was significant difference in the ADC values of the medial and lateral gastrocnemius muscles between the first and second degree injury groups(P<0.05). However, there was no difference in FA values(P>0.05). 【Conclusion】 DTI can be used for quantitative analysis of gastrocnemius muscle after injury, and its parameters are related to the degree of injury, which can provide quantitative basis for the evaluation of gastrocnemius muscle
