摘要
【目的】观察乌司他丁对老年脊柱手术患者术后炎性反应和疼痛的作用。【方法】选择首都医科大学宣武医院2016年2月至2017年2月老年患者80例做腰椎减压+内固定术的,采用随机对照原则分为对照组(C组)和乌司他丁组(U组)各40例。乌司他丁组在切皮前静脉输注乌司他丁10000 U/kg,术后第1天和第2天静脉输注乌司他丁5000 U/kg;对照组给予生理盐水安慰剂。观察切皮前(T0)、术后4 h(T1)、术后24 h(T2)及术后72 h(T3)静息和活动时视觉模拟评分(visual analogue score,VAS)、镇痛泵使用情况并检测各时间点血清C-反应蛋白、白介素-6和脂多糖的浓度。【结果】乌司他丁可显著降低老年脊柱手术老年患者术后疼痛评分,减少阿片类药用量,并且乌司他丁组患者术后血清C-反应蛋白、白介素-6和脂多糖浓度水平显著低于对照组患者(P<0.05)。【结论】乌司他丁干预可以减轻老年脊柱手术患者的术后炎性反应,缓解患者术后疼痛。
【Objective】To observe the effect of ulinastatin on postoperative inflammatory response and pain in elderly patients undergoing spinal surgery.【Methods】A total of 80 elderly patients undergoing lumbar decompression and internal fixation were selected in Xuanwu Hospital of Capital Medical University from February 2016 to February 2017.They were randomly divided into control group(group C)and ulinastatin group(group U),40 cases per group.The patients in the group U received 10000 U/kg of ulinastatin intravenously before skin cutting,and 5000 U/kg of ulinastatin intravenously on the first and second day after operation.Those in the group C received 0.9%normal saline as placebo.The visual analogue score(VAS),serum C-reactive protein and interleukin-6 concentration were observed before skin incision(T0),4 hours after operation(T1),24 hours after operation(T2),72 hours after operation(T3).Opioid consumption was observed at 4,24,48 hours after operation.The difference was statistically significant as follows.(P<0.05).【Results】Ulinastatin could significantly reduce the postoperative pain score in elderly patients undergoing spinal surgery and opioid consumption.The serum levels of C-reactive protein and interleukin-6 concentration in group U were significantly lower than those in the group C(P<0.05).【Conclusion】Ulinastatin can reduce the postoperative inflammatory response and pain in elderly patients undergoing spinal surgery.
