摘要
目的探析CT、MRI动态增强诊断双侧肾上腺非霍杰金淋巴瘤的临床价值。方法对本院2017年1月—2020年1月收治的12例肾上腺非霍杰金淋巴瘤患者的临床资料进行回顾性分析,所有患者均采取CT结合MRI动态增强扫描。观察分析病变形态、大小、边缘和强化程度及是否合并腹膜后淋巴结肿大。结果12例患者共24个病灶,其中肿块形8个,椭圆形12个,肾上腺增生改变4个;CT平扫提示密度均匀20个病灶,密度不均匀4个,其中病灶内坏死2个;CT动态增强动脉期病灶呈轻度强化,20个病灶均匀强化,4个不均匀强化,且可见网格状、条状强化;MR扫描T1WI信号呈稍低信号,T2WI呈稍高信号,且增强扫描能见静脉期持续强化,动脉期轻度强化;合并腹膜后淋巴结肿大6例;临床医师确诊8例,误诊4例。结论CT、MRI动态增强用于肾上腺非霍杰金淋巴瘤诊断中呈轻度到中度持续性延迟强化,且较为均匀,采用CT及MRI动态增强扫描有助于确诊,为临床治疗方案的制定提供可靠信息。
Objective To explore the clinical value of dynamic contrast-enhanced CT and MRI in the diagnosis of bilateral adrenal non Hodgkin's lymphoma.Methods The clinical data of 12 patients with adrenal non Hodgkin's lymphoma treated in Xinyang Central Hospital from January 2017 to January 2020 were retrospectively analyzed.All patients were taken CT plain scan and dynamic enhanced scan.The shape,size,margin and enhancement degree of the lesions were determined,and whether retropeRIToneal lymphadenopathy was complicated was analyzed.Results There were 24 lesions in 12 patients,including 8 masses,12 ellipses,and 4 adrenal hyperplasia changes;CT scan showed 20 lesions with uniform density,4 lesions with uneven density,and 2 lesions with necrosis in the lesions;the lesions in arterial phase showed mild enhancement,20 lesions showed uniform enhancement,4 inhomogeneous enhancement,and grid and strip enhancement could be seen;MRI showed slightly low signal intensity on T1WI There were 6 cases with retropeRIToneal lymphadenopathy,8 cases were diagnosed by clinicians and 4 cases were misdiagnosed.Conclusion Dynamic contrast-enhanced CT and MRI showed mild to moderate delayed enhancement in the diagnosis of adrenal non Hodgkin's lymphoma,and it was more uniform.Dynamic contrast-enhanced CT and MRI diagnosis is helpful to the diagnosis and provide reliable information for the formulation of clinical treatment plan.
