Epidemiological and etiological characteristics of influenza in Nanjing from 2011 to 2020

摘要 目的分析2011-2020年南京市流行性感冒(简称流感)流行趋势,了解近年来南京地区流感的流行规律和特征。方法采用实时荧光定量PCR法对2011-2020年南京市3家哨点医院(南京市儿童医院、南京市第一医院、南京市鼓楼医院)流感病例样本进行检测并分型。用Excel2007进行病例资料整理,SPSS 13.0进行数据分析。结果2011-2020年共收检疑似流感病例28847例,其中核酸阳性例数3941例,阳性率为13.66%,阳性样本中A型流感2466份,以H3N2亚型为主,占A型流感总阳性数的65.17%(1607/2466),其次为新甲H1N1亚型(34.27%);B型流感1442份。每年12月至次年3月为发病高峰,占全年阳性数的57.42%(2263/3941)。患者以0~14岁人群为主,占总阳性数的42.78%,不同年龄组人群阳性率差异有统计学意义(χ^(2)=183.493,P<0.05);男性与女性发病率差异无统计学意义(P<0.05)。结论2011-2020年南京市流感以新甲H1N1、H3N2亚型和B型为主要优势毒株。应加强对各年龄人群的疫苗接种工作,并加大对学校等人群聚集处的防控管理力度。 Objective To analyze the influenza epidemic trend in Nanjing from 2011 to 2020 and provide a basis for influenza prevention and control.Methods Influenza samples were collected from influenza like illness(ILI)patients in three sentinel hospitals(Nanjing Children’s Hospital,Nanjing First Hospital,Nanjing Drum Tower Hospital),the positive cases and influenza subtype were determined by reverse transcriptase polymerase chain reaction.Excel2007 and SPSS 13.0 were used to analyze the data.Results A total of 28847 suspected influenza cases were collected from 2011 to 2020,of which 3941 were influenza positive,with a detection rate of 13.66%;the cases of type A were 2466,type B were 1442.Type A was dominated by H3N2 subtype,accounting for 65.17%(1607/2466),followed by 2009 pandemic H1N1(34.27%).The Nanjing influenza peaks were mostly from December to March each year,accounting for 57.42%(2263/3941);the majority of patients were 0-14 years old(42.78%).The positive rate of different age groups was statistically significant(χ^(2)=183.493,P<0.05),and there was no significant difference in the incidence rate between male and female(P<0.05).Conclusions The influenza epidemic in Nanjing from 2011 to 2020 was seasonal;the dominant subtypes were2009 pandemic H1N1,H3N2,and influenza B.Vaccination should be carried out between October and December of each year for all ages,and the prevention and control of influenza in schools and other public places should be strengthened.
