摘要
目的研究肾移植受者术后BK病毒(BKV)感染与淋巴细胞计数的相关性。方法选取2015年3月-2020年7月于郑州市第七人民医院收治的498例肾移植受者,根据病毒感染检测结果分为BKV阴性组(n=409)和BKV阳性组(n=89),采用定量PCR检测肾移植受者的BKV DNA载量,比较不同感染结果患者的淋巴细胞计数,采用ROC曲线分析淋巴细胞计数对BKV感染的诊断价值,利用Spearman法分析肾移植受者外周血BKV DNA载量与淋巴细胞计数的相关性。结果498例肾移植受者中,89例患者外周血中定量检测出BKV,阳性检出率为17.78%。BKV血症阳性患者BKV DNA载量高于阴性患者(P<0.05);BKV血症阴性患者CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)值高于BKV血症阳性患者(P<0.05)。CD4^(+)/CD8^(+)诊断BKV血症的AUC为0.840(95%CI:0.707~0.929,P<0.001)。肾移植受者外周血BKV DNA定量水平与CD8^(+)淋巴细胞计数呈正相关(r=0.498,P<0.05)。结论淋巴细胞计数可有效诊断肾移植受者术后BKV感染,且CD8^(+)淋巴细胞计数与BKV DNA定量水平呈正相关。
Objective To study BK virus(BKV)infection and the correlation between its quantitative level and lymphocyte count in patients after renal transplant.Methods A total of 498 renal transplant patients who were admitted to the Seventh People’s Hospital of Zhengzhou from March 2015 to July 2020 were enrolled.According to the test results of virus infection,they were divided into BKV-negative group and BKV-positive group.The loads of BKV DNA in renal transplant patients were detected by quantitative PCR.The lymphocyte counts in patients with different infection results were compared.The diagnostic value of lymphocyte count for BKV infection was analyzed by ROC curves.The correlation between BKV DNA load in peripheral blood and lymphocyte count was analyzed by Spearman method.Results Among the 498 renal transplant patients,there were 89 cases(17.78%)with BKV in peripheral blood.The levels of BKV DNA in patient with peripheral blood BKV-positive were higher than those with peripheral blood BKV-negative(P<0.05).The levels of CD4^(+),CD8^(+)and CD4^(+)/CD8^(+)in patient with peripheral blood BKV-negative were higher than those with peripheral blood BKVpositive(P<0.05).AUC value of CD4^(+)/CD8^(+)for the diagnosis of peripheral blood BKV was 0.840(95%CI:0.707-0.929,P<0.001).The quantitative levels of BKV DNA in peripheral blood was positively correlated with CD8^(+)lymphocyte count in renal transplant patients(r=0.498,P<0.05).Conclusion The CD8^(+)lymphocyte count could effectively diagnose BKV infection in patients after renal transplant,which was positively correlated with the quantitative level of BKV DNA.
