摘要
目的:观察中医熏蒸联合针灸对行开放楔形胫骨高位截骨手术的膝关节骨关节炎患者手术指标及疗效的影响。方法:选取我院2016年3月至2018年3月收治的84例膝关节骨关节炎患者,按照随机数表法分为对照组和观察组,各42例。对照组给予采用OWHTO,观察组在对照组基础上接受中医治疗。术后比较两组患者术中及预后情况,统计膝关节功能变化情况,观察下肢机械力线变化以及治疗效果。结果:对照组患者的手术时间、透析次数、出血量等与观察组对比未见显著差异(P>0.05)。对照组患者的HSS评分和WOMAC评分低于观察组,对比差异有统计学意义(P<0.05)。对照组患者HKA角小于观察组,对比差异有统计学意义(P<0.05)。对照组患者临床治疗的总有效率88.10%低于观察组97.62%,对比差异有统计学意义(P<0.05)。结论:在膝关节骨关节炎患者行开放楔形胫骨高位截骨术时,采用改良单平面截骨及下肢机械力线矫正方法并联合使用中医疗法进行干预,具有较好的临床治疗效果,能改善患者的膝关节功能,提高患者的临床治疗效果,值得推广和应用。
Objective: To investigate the effect of traditional Chinese medicine fumigation combined with acupuncture on surgical indexes and curative effect of knee osteoarthritis patients undergoing open wedge-shaped tibial high osteotomy. Methods: A total of 84 patients with knee osteoarthritis admitted to our hospital from March 2016 to March 2018 were selected and divided into a control group and an observation group according to a random number table, with 42 cases in each group. The control group was given OWHTO, and the observation group received TCM treatment on the basis of the control group. After the operation, the patients in the two groups were compared in the intraoperative and prognosis, the changes of knee joint function were counted, the changes of mechanical force line of the lower extremity and the treatment effect were observed. Results: t There was no significant difference between the control group and the observation group in terms of operation time, dialysis times, blood loss, etc.(P>0.05). The scores of HSS and WOMAC in the control group were lower than those in the observation group(P< 0.05). The HKA angle of the control group was smaller than that of the observation group(P< 0.05). The total effective rate of the control group was 88.10% lower than that of the observation group(97.62%)(P< 0.05). Conclusion: when open wedge tibial osteotomy is performed in patients with osteoarthritis of the knee, the improved single plane osteotomy and the mechanical force line correction of the lower extremities are effective in clinical treatment, which is worth popularizing.
