摘要
目的:观察加减五苓散联合西医治疗肾病综合征水肿对患者炎症因子、水肿程度的影响,为临床治疗提供参考。方法:选取我院2016年11月~2019年4月收治的肾病综合征水肿患者110例,根据治疗方法分为两组,每组55例。西医组给予西医常规对症治疗,联合组在西医组基础上给予加减五苓散治疗。记录两组治疗前后24h尿量(24h-UV)、体重、炎症因子、24h尿蛋白定量、血清白蛋白(Alb)、血肌酐(Scr)、胱抑素C(CysC)水平变化,并观察两组临床治疗总有效率、水肿消退时间。结果:治疗后,联合组总有效率(89.09%)高于西医组(74.55%),水肿消退时间(9.94±2.18)d短于西医组(11.41±3.25)d,差异具有统计学意义(P<0.05)。治疗后,两组24h-UV、Alb水平较治疗前上升,体重、白细胞介素-8(IL-8)、白细胞介素-18(IL-18)、肿瘤坏死因子-α(TNF-α)、转化生长因子-β(TGF-β)、24h尿蛋白定量水平较治疗前下降,Scr、CysC水平较治疗前比较,差异不具有统计学意义(P>0.05);联合组24h-UV、Alb水平高于西医组,体重、炎症因子、24h尿蛋白定量水平低于西医组,Scr、CysC水平与西医组比较,差异不具有统计学意义(P>0.05)。结论:加减五苓散联合西医治疗肾病综合征水肿可有效改善水肿程度,减轻炎症反应,保护肾功能。
Objective:To observe the effect of Modified Wuling Powder combined with western medicine on the inflammatory factors and the degree of edema for patients with nephrotic syndrome,and to provide reference for clinical treatment.Methods:110 patients with edema of nephrotic syndrome treated in our hospital from November 2016 to April 2019 were divided into two groups according to the treatment method,55 cases in each group.The western medicine group was treated with routine symptomatic treatment of western medicine,and the combined group was treated with Modified Wuling Powder on the basis of western medicine group.The 24-hour urine volume(24 h-UV),body weight,inflammatory factor,24-hour urine protein,serum albumin(Alb),blood myoglobin(Scr)and cysC(cysC)were recorded in 2 groups.Results:After treatment,the total effective rate of the combined group(89.09%)was higher than that of the western medicine group(74.55%).The regression time of edema(9.94±2.18)d was shorter than that of the western medicine group(11.41±3.25)d,and the difference was statistically significant(P<0.05).After treatment,the levels of 24-hour-UV and Alb in the 2 groups were higher than those before treatment,and the levels of body weight,Interleukin-8(IL-8),IL18(IL-18),tumor necrosis factor(TNF-α),transforming growth factor Beta(TGF-β)and 24-hour-UV were lower than those before treatment.The levels of Scr and CysC were not significantly different from those before Treatment(P>0.05),the levels of 24 h-UV and Alb in the combined group were higher than those in the western medicine group,the levels of body weight,inflammatory factors and 24-hour urine protein were lower than those in the western medicine group,the levels of Scr and CysC were lower than those in the western medicine group.The difference was not statistically significant(P>0.05).Conclusion:Modified Wuling Powder combined with western medicine can effectively improve the degree of edema,reduce inflammatory reaction and protect renal function.
